Aim:Behavioral and psychosocial factors have received more and more attention in the occurrence,prevention,and treatment of atherosclerosis and cardiovascular disease(CVD).Sleep disorder is an important behavioral factor in the occurrence of CVD,and insomnia is the most common sleep disorder.This paper aims to clarify the occurrence and correlation of atherosclerosis in insomnia patients.Methods:Collect 60 patients with insomnia who were diagnosed and treated in the outpatient department of general medicine or inpatient department of Chaohu Hospital affiliated to Anhui Medical University from December 2020 to November 2021 and can complete the Pittsburgh Sleep Quality Index and Insomnia Severity Index.According to the duration of insomnia,60 patients with insomnia were divided into chronic insomnia group(duration≥3 months)and short-term insomnia group(duration<3 months).In addition,58 healthy people who were examined in the physical examination center of Chaohu Hospital affiliated to Anhui Medical University in the same period were selected as the control group.Collect the general clinical data,blood glucose,blood lipids and other biochemical indicators of the two groups.Brachial-ankle pulse wave velocity(ba PWV)was measured with Omron BP203RPEIII arteriosclerosis detector to assess arterial stiffness.Carotid artery intima-media thickness(CIMT)and plaque incidence were examined with color Doppler ultrasound.These indicators are used to determine the occurrence and extent of carotid atherosclerosis.Two independent sample t-tests were used to compare the general clinical data,biochemical,ba PWV,CIMT and plaque incidence of insomnia group and control group.The indexes of ba PWV,CIMT and plaque incidence were compared between chronic insomnia group and short-term insomnia group.Multiple linear regression was used to analyze the effects of insomnia,blood pressure,blood glucose,blood lipids,BMI,and age on atherosclerosis.All data were analyzed using SPSS 20.0 statistical software,and the difference was statistically significant with P<0.05.Results:1.There was no significant difference in sex,age,SBP,DBP,FBG,TG,TC,LDL-C,Hcy,BMI and smoking rate between the insomnia group and the control group(P>0.05),but the incidence of ba PWV,CIMT and carotid plaque in the insomnia group was higher than that in the control group(ba PWV:1412.33±141.80VS 1256.21±140.00,t=6.017,P<0.001;L-CIMT:0.11±0.03 VS 0.07±0.02,t=6.996,P<0.001;R-CIMT:0.12±0.04 VS 0.07±0.02,t=8.736,P<0.001;Incidence of carotid plaque:31.7%VS 19.0%,χ~2=1.162,P=0.023).2.The incidence of ba PWV,CIMT and carotid plaque in chronic insomnia group was higher than that in short-term insomnia group(ba PWV:1465.00±142.58 VS 1333.33±98.94,t=3.932,P<0.001;L-CIMT:0.12±0.03 VS 0.10±0.03,t=2.473,P=0.016;R-CIMT:0.13±0.03 VS 0.11±0.04,t=2.573,P=0.013;Incidence of carotid plaque:36.1%VS 25.0%,χ~2=1.101,P=0.041).3.Multiple linear regression was used to analyze the effects of insomnia,blood pressure,blood glucose,blood lipids,BMI and age on atherosclerosis.The results showed that the increase of CIMT in atherosclerosis group was significantly related to insomnia and age(insomnia:left R2=0.27,L-CIMT Beta=0.462,t=4.342,P<0.001;right R~2=0.37,R-CIMT Beta=0.526,t=5.339,P<0.001;age:L-CIMT Beta=0.235,t=2.082,P=0.029;R-CIMT Beta=0.203,t=1.728,P=0.038).There was no correlation between blood pressure,blood glucose,blood lipids,BMI and the increase of CIMT(P>0.05).Conclusion:1.Insomnia is associated with atherosclerosis,and with the prolongation of the duration of insomnia,the correlation between the two becomes more obvious.2.Age is related to the occurrence of atherosclerosis.The older the age,the higher the risk of atherosclerosis.3.Through the detection of ba PWV,CIMT and carotid plaque,atherosclerosis can be detected and diagnosed at an early stage,providing the possibility for early intervention of atherosclerosis and avoiding the occurrence of more serious cardiovascular and cerebrovascular diseases. |
