Background:Hypertension is an important modifiable cardiovascular risk factor with a high prevalence.Home blood pressure measurement,office blood pressure measurement and ambulatory blood pressure measurement are three important methods of blood pressure measurement,while home blood pressure monitoring has its own characteristics and advantages.In the past 20 years,home blood pressure monitoring has been continuously developed and become a means of blood pressure monitoring recommended by hypertension management guidelines in various countries and regions,and the awareness and use of home blood pressure monitoring has increased dramatically.Although blood pressure measured in the clinical office setting often determines treatment decisions,there is growing evidence that the quality and accuracy of clinical measurements vary widely and that a variety of factors can contribute to inaccurate blood pressure readings taken in the office,including improper patient positioning,improper blood pressure measurement techniques,And white coat hypertension and latent hypertension are inevitable in the process of blood pressure measurement in the consulting room.Accurate measurement of blood pressure is an important part of the treatment and management of hypertension.Many studies have shown that home blood pressure monitoring is better than clinic blood pressure in predicting cardiovascular and other target organ damage.Nevertheless,home blood pressure monitoring is still in its infancy in China,and it is important to raise awareness of home blood pressure monitoring among both healthcare personnel and patients.Hypertension is one of the important risk factors of atherosclerosis,which is an important pathological basis of target organ damage.Early detection and diagnosis of atherosclerosis can slow down the occurrence and development of the disease.Objective: To investigate the association between home blood pressure monitoring,office blood pressure monitoring and atherosclerosis.Methods: A total of 247 patients with hypertension were included.Coronary angiography was performed,syntax score was calculated,peripheral arteriosclerosis test of extremities was underwent,ABI and ba PWV were recorded,and other clinical data were recorded,to compare the difference between home blood pressure monitoring and office blood pressure monitoring.The association between home blood pressure,office blood pressure and syntax score,ABI,ba PWV was analyzed,and the association between home blood pressure,office blood pressure and atherosclerosis was compared.Results: There were statistically significant differences between home Sbp and office Sbp(P< 0.05),and there was no significant difference between home Dbp and office Dbp(P>0.05);home Sbp was strongly correlated with syntax score,ABI and ba PWV(r=0.779,0.710,0.731,P<0.05),home Dbp was weakly correlated with syntax score,ABI and ba PWV(r=0.170,0.240,0.238,P<0.05),office Sbp was weakly correlated with syntax score,ABI and ba PWV(r=0.191,0.218,0.188,P<0.05),office Dbp was weakly correlated with syntax score,ABI and ba PWV(r=0.187,0.189,0.238,P<0.05).The association between home Sbp and syntax score,ABI and ba PWV was much greater than that between home Dbp,office Sbp and office Dbp and syntax score,ABI and ba PWV respectively.Conclusions: home Sbp is more associated with coronary and peripheral atherosclerosis and is an independent risk predictor of vascular atherosclerosis.
