Objective Fibromyalgia is an important concomitant symptom of inflammatory arthropathy,but compared with the results of investigations carried out by foreign researchers in different regions,there is still a lack of epidemiological data describing the combination of fibromyalgia and inflammatory arthropathy in China.In this study,by combining the diagnosis and treatment files of clinical departments with on-the-spot investigation data,we can understand the current situation and population characteristics of inflammatory arthropathy complicated with fibromyalgia,and explore the possible influencing factors of this kind of combination.To provide scientific basis for improving the quality of clinical diagnosis and treatment of patients with arthropathy complicated with fibromyalgia.Methods A cross-sectional study design was used to investigate the patients who were diagnosed as inflammatory arthropathy in the outpatient and inpatient centers of the rheumatic immunology department.Through the on-the-spot questionnaire survey and the past electronic medical record inquiry,the general situation,behavior habits,diseases history,patient-related clinical examination indicators and treatment medication of the subjects were collected,as well as the corresponding disease activity and function score and the current self-pain,fatigue,mood and sleep quality test scores.The database was established by Excel and Epi Data3.1 software,and the data was analyzed and processed by SPSS 23.0 and R software.Results A total of 396 patients with inflammatory arthropathy were investigated,of which 286 patients were mainly diagnosed with rheumatoid joints,and the remaining 110 patients were diagnosed with axial spondyloarthropathy.There were 120 males and 276 females with an average age of(50.45 ± 15.44)years.Analysis of influencing factors of inflammatory arthropathy combined with fibromyalgia: The results showed that the two groups of patients with inflammatory arthropathy combined with fibromyalgia had significant differences in patient source,age,education level,occupation type,household registration type,per capita monthly income,average monthly expenditure,missed days due to illness,disease history,fracture history,milk and dairy product consumption and physical exercise(P < 0.05).Compared with the non-pooled group,54.1% of the patients in the pooled group were mainly from inpatient surveys.The average age was 55.3 years old;the proportion of primary school education and below was higher at 62.3%;The types of occupations were mainly unemployed and retirees,accounting for 72.9%;The proportion of rural hukou is as high as 82.4%;Per capita monthly income is relatively lower and average monthly expenditure is relatively higher;72.9% of the number of missed work and school days due to medical treatment were also increased;In the past,the proportion of people diagnosed with other diseases was as high as 78.9%,especially chronic cardiovascular and cerebrovascular diseases,which accounted for the largest proportion of 32.9%;The proportion of people with a history of fracture in the past(31.8%)was higher than that in the non-pooled group(18.3%).The proportion of people who are daily dairy-free and dairy drinking habits(47.1%)and never do any physical activity(81.2%)is also higher.The prevalence of inflammatory arthropathy complicated with fibromyalgia tended to decrease with the increase of patients’ education level,per capita monthly income,milk and dairy product consumption and physical activity.The trend increases with average monthly expenditure and lost work and school days.The results of multivariate analysis showed that rural hukou(OR value of 2.10),unemployed/retired persons(OR value of 2.24),missed work and school due to different days of illness(1-2 days OR value was 3.99,2-4 days OR value was 6.84,and OR value greater than 4 days was 4.65),and previous fracture experience(OR value was 1.84)were risk factors for inflammatory joint disease combined with fibromyalgia.Further analysis and comparison of clinical diagnosis and treatment methods,fatigue,mood and sleep conditions of the two groups showed that compared with the nonconcombinant group,the proportion of glucocorticoid drugs used in inflammatory arthropathy patients with fibromyalgia was higher at 48.2%.The use of non-steroidal antiinfl ammatory drugs(NSAIDs)was even lower,at 21.2%;Disease activity scores,pain,fatigue,mood,and sleep scale scores were significantly higher and statistically significant(P < 0.05).Patients in the pooled group tended to exhibit more intense disease activity and painful experiences,more severe fatigue and negative mood,and poorer sleep quality.Conclusion According to the investigation and analysis of this study,the prevalence of inflammatory arthropathy complicated with fibromyalgia is about 20%.Compared with patients with simple inflammatory arthropathy,patients with inflammatory arthropathy with fibromyalgia showed lower education level,lower income level,previous diagnosis of other diseases and fractures,lower levels of physical activity,higher disease activity scores,more use of hormonal drugs,prominent pain and fatigue,significant negative mood,and worse sleep quality.
