Objective: To investigate the clinical characteristics of 217 patients with CMLCP in our center,and to analyze the prognosis and construct a nomogram.Methods:1.The clinical characteristics of 217 newly diagnosed CML-CP patients in our center from July 2007 to April 2022 were retrospectively analyzed.COX regression analysis was used to analyze the impact of Overall Survival(OS)and Progression-free Survival(PFS)and Event-free Survival(EFS)were independent prognostic risk factors,and log-rank test was used to test the difference in survival rate among subgroups.2.The influence of Additional chromosomal aberrations(ACAs)on prognosis was analyzed by Kruskal-Wallis non-parametric test or chi-square test.3.Build the nomogram prediction model of OS and PFS through R language and conduct internal verification.Result:1.The median age of onset was 45(4-90)years,The proportion of male: female patients was 1.55,85 patients had comorbidities,more patients with low Sokal score and ELTS score,78 patients(35.9%)with additional chromosomal abnormalities and kinase region mutations,and the most patients were treated with imatinib,followed by dasatinib,reaching DMR in 69 patients(31.8%).The median OS was 60 months,median PFS was 52 months,and median EFS was 48 months.2.COX regression analysis showed that age,comorbidities and MR5.0 were independent prognostic factors for OS.Age,HGB and use of dasatinib were independent prognostic factors for PFS.Age and use of dasatinib were independent prognostic factors for EFS.3.After Propensity score matching(PSM),CML-CP patients with ACAs were analyzed to achieve Complete cytogenetic response(CCy R),MR5.0,Deep molecular response(DMR)ratio and OS rate were significantly reduced.4.The nomogram model of OS showed that the AUC values of 3-year,5-year and 10-year OS were 0.784,0.842 and 0.834,respectively,which had good discrimination and accuracy,and had clinical benefits within a certain threshold range.The nomogram model of PFS showed that the AUC values of 3-year,5-year and 10-year PFS were 0.661,0.733 and 0.696,respectively,and the accuracy and clinical benefit of 10-year PFS were good.Conclusion:1.Age at diagnosis,comorbidities and HGB level are independent prognostic factors for the survival of CML-CP patients.The use of dasatinib and the achievement of DMR have a positive impact on patient outcomes.2.CML-CP patients with ACAs have poor survival.3.The OS nomogram of CML-CP patients has good predictive ability.
