Objective This study analyzed the spectrum of legal infectious diseases in Gansu Province from 2015 to 2020 and the basic situation of treatment costs for infectious and parasitic diseases in Gansu Province in 2020,and explored the trend of direct economic burden changes for patients with infectious and parasitic diseases from2015 to 2020.Based on the sample database,multiple linear regression,neural network model,and random forest model are constructed to analyze the influencing factors of hospitalization expenses,providing a basis for government departments to accurately implement policies.Methods Based on the theoretical framework of SHA2011,multi-stage stratified sampling was used for data collection,and data accounting was conducted through STATA 14.0;The collected case information of patients with infectious and parasitic diseases will be formed into a sample library,and H test and Spearman rank correlation test will be conducted through STATA 14.0;SPSS 25.0 was used to conduct single factor analysis,multiple linear regression analysis,and neural network model construction.PYTHON 3.6.0 was used to construct a random forest model to analyze the influencing factors of hospitalization expenses.Results 1.The top five infectious diseases in Gansu Province during 2015-2020 were viral hepatitis,other infectious diarrhea,tuberculosis,hand foot mouth disease and influenza.Respiratory tract infections account for the highest incidence rate,and digestive tract infections rank second.2.In 2020,the total treatment cost for infectious and parasitic diseases in Gansu Province was 996168300 yuan,with outpatient treatment costs of 443.603 million yuan.The treatment cost for males was higher than that for females.The treatment cost for patients aged 45 to 64 is high,and the treatment cost for patients aged 0 to 4also exceeds 50 million yuan.The top three diseases with total treatment costs are viral infection,viral hepatitis and tuberculosis characterized by skin and mucosal lesions;Among them,the top ten in total treatment costs for infectious diseases are19.43-231.7712 million yuan.The overall treatment cost for parasitic diseases is lower than that for infectious diseases,and the highest treatment cost is 3.8899 million yuan for helminth diseases.The main body of treatment costs is hospitals,accounting for 92.17%,and mainly comprehensive hospitals.93.78% of hospitalization expenses occur in hospitals.In 2020,the main treatment providers for infectious and parasitic diseases were municipal medical institutions,with county-level institutions accounting for only 13.32%.3.During the period from 2015 to 2020,the proportion of male and female patients seeking medical treatment was1.27:1,with the largest proportion of hospitalized patients aged 0 to 4 years.62.89%of patients choose urban and rural medical insurance settlement to pay for hospitalization expenses,while 22.72% of patients still choose to pay at their own expense.The largest number of patients choose to be hospitalized in municipal medical institutions,accounting for 38.16%,and 32.84% choose to be hospitalized in grassroots medical institutions.4.During the period from 2015 to 2020,the average hospitalization days of patients with infectious and parasitic diseases showed a downward trend,but the average hospitalization expenses per patient and daily hospitalization expenses increased.The treatment fee and examination fee are positively correlated with the year;Drug and surgical expenses are negatively correlated with year.The top three diseases of average cost per case were worm disease,AIDS disease and tuberculosis.5.Surgery and length of stay are the most important factors that affect the hospitalization expenses of hospitalized patients,and institution type and age also have a significant impact.Conclusion 1.The high prevalence of viral hepatitis and tuberculosis in Gansu Province has caused a serious economic burden in terms of treatment costs,and should be given special attention.The overall control of parasitic diseases is good and should continue to be maintained.2.The cost of treating infectious and parasitic diseases in Gansu Province is higher for males than for females,while hospitalization is higher than outpatient services.Patients in the age group of 45 to 65 years old have the largest body size and highest cost,and the cost mainly flows to general hospitals and county-level institutions.The allocation of health resources in Gansu Province needs to be further strengthened.3.As the direct medical burden on patients increases,cost control should be implemented,and control over drug and treatment expenses should be strengthened.Although the total cost of parasitic diseases is relatively small,the personal treatment cost for hospitalized patients is high.In the future,further efforts should be made to strengthen the control of emerging infectious diseases.4.Surgery and length of stay are the most important factors affecting the hospitalization expenses of hospitalized patients.Strengthening prevention and control of infectious and parasitic diseases should be carried out to reduce the demand for surgery.By regulating the behavior of hospitals and doctors,unnecessary expenses can be controlled. |
