Clinical Observation On The Treatment Of Painful Diabetic Peripheral Neuropathy With Glucose-Lowering Pair Of Prescriptions Combined With Addition And Reduction Of Danggui Sihui Tang And Western Medicine

Objective In this study,a combination of Chinese herbal soup and methylcobalamin tablets was used for clinical application in patients with painful diabetic peripheral neuropathy(qi-yin deficiency and cold clotting and blood stasis),and its safety and efficacy were observed in order to provide clinical treatment ideas for patients with painful diabetic peripheral neuropathy.MethodsFrom 60 patients suffering from painful diabetic peripheral neuropathy(Qi-Yin deficiency,cold clotting and blood stasis type),30 cases were randomly selected as the control group and 30 cases as the observation group for further study.Patients in both groups first received the same basic treatment for diabetes mellitus,then the control group received only methylcobalamin tablets,while the observation group was treated with the hypoglycaemic pair of prescriptions combined with danggui siwei tang plus reduction and methylcobalamin tablets,and both groups were treated for 8 weeks.Comparative analysis of the patients’ median nerve,tibial nerve motor and sensory nerve conduction velocities,Toronto clinical scores and TCM symptom scores led to the conclusion.Results(1)Clinical efficacy: the total effective rate of 86.7% in the observation group was significantly greater than that of 66.7% in the control group,with statistical P<0.05.(2)Nerve conduction velocity: after treatment,the conduction velocity of MNCV and SNCV in the two groups increased significantly P<0.05;the increase of nerve conduction velocity in the observation group was better than that in the control group P<0.05.(3)TCSS scores: compared with before treatment,the scores of the observation group and the control group decreased after treatment(P<0.01),indicating that the treatment regimens of both the observation group and the control group were effective,and compared with the control group,the score of the observation group decreased more significantly after treatment(P<0.05),which indicated that the regimen of hypoglycemic pair of prescriptions combined with danggui siwei tang plus and minus combined with mecobalamin was better than the regimen of mecobalamin alone.(4)TCM syndrome scores: after treatment,the TCM syndrome scores of the patients were compared within the group at P<0.01,but the total TCM syndrome scores of the patients in the two groups were compared between the groups at P<0.05,and the observation group was better than the control group;and the overall effective rates of the observation group and the control group were 90.0% and 73.3% respectively,which were significantly higher than those of the control group.(5)No adverse reactions occurred in the 2 groups before and after treatment,with good safety.ConclusionsIn patients with painful diabetic peripheral neuropathy(Qi-Yin deficiency,cold clotting and blood stasis type),the use of the hypoglycaemic counterpart formula combined with Danggui Sihui Tang plus reduction and supplemented with methylcobalamin was significantly more effective than the use of methylcobalamin alone.After the study,it was found that the regimen of Chinese herbal soup combined with methylcobalamin for the treatment of painful diabetic peripheral neuropathy had minimal side effects,and no negative effects were found during the trial phase,and the clinical patients agreed highly with this therapy,which is worthy of application in medical practice.
