Satisfaction With Pharmacotherapy In Outpatients With Chronic Obstructive Pulmonary Disease And Its Affecting Factors:A Cross-Sectional Study

Purpose: Long-term standardized medication is an important factor in the successful treatment of chronic obstructive pulmonary disease(COPD),but patients with COPD have poorer treatment adherence,and studies have shown that patient satisfaction with treatment is related to adherence to treatment.This study aimed to investigate the treatment satisfaction level of patients with COPD and to explore the underlying factors associated with patient satisfaction.Methods: A cross-sectional study was conducted in ambulatory patients with COPD at the Second Xiangya Hospital,Changsha,Hunan,between August 1,2020,and May 31,2021.The Treatment Satisfaction Questionnaire for Medication version Ⅱ(TSQM v.Ⅱ)was used to assess the treatment satisfaction of patients.Multivariable regression analysis was used to determine the factors associated with treatment satisfaction.Results: 392 ambulatory patients with COPD first participated in the study,and 354 patients with are finally available.Median(IQR)score in effectiveness was 66.67(50.00,66.67),in side effects was 100(100,100),in convenience was 66.67(66.67,66.67),in global satisfaction was66.67(50.00,66.67).Compared with the satisfied group,the patients who were unsatisfied with the treatment had more acute exacerbations in the past year(1 vs 0,P<0.001),higher CAT scores(13 vs 10,P<0.001),higher CCQ scores(2.24 vs 1.95,P < 0.001),higher GOLD group classification(P < 0.001)and higher BODEx(P=0.006).38.7% of the participants were dissatisfied with the effectiveness of the inhaled preparation,38.4% were dissatisfied with the overall response,91.2%were satisfied with the adverse reactions,and 83.1% were satisfied with the convenience.Results indicate that different inhaled preparation didn’t impact the satisfaction with treatment.The number of exacerbations in the past 1 year,Number of acute exacerbations in the past year,GOLD group,scores of CAT,BODEx were correlated with global satisfaction(r=-0.234,-0.240,r=-0.239,r=-0.206,r=-0.139,all P <0.05).The multivariable regression analysis showed that global satisfaction was associated inversely with CAT scores(β=-0.372,P =0.018)and CCQ scores(β=-2.636,P =0.041).Conclusion: COPD patients did not have high satisfaction scores for inhaled preparation therapy.The severity of a patient’s respiratory symptoms and quality of life are the main factors influencing patient satisfaction and these should be taken into account in developing a personalized treatment plan for patients.
