Present Situation And Influencing Factorsof Bowel Preparation Quality In Patients Before Colonoscopy

Objective:This study aimed to evaluate the present situation of bowel preparation quality and to explore the influencing factors of bowel preparation quality among patients before colonoscopy by collecting perioperative information from colonoscopy patients.Thus,to provide scientific proposal for the improvement of bowel preparation quality before colonoscopy.Methods:From 1 September 2021 to 20 January 2022,a study using convenience sampling method was conducted at the Gastrointestinal Endoscopy Center of Xiangya Hospital,Central South University.Patients who undergo colonoscopy and meet the inclusion criteria were recruited.Information on the basic situation,disease-related factors and preparation factors of this colonoscopy was collected using a questionnaire.Boston Bowel Preparation Scale was used to evaluate patients’intestinal cleanliness before colonoscopy.?~2test was used for the comparison between groups.Binary logistic regression analysis was used for multivariate analysis.Results:1.From 1 September 2021 to 20 January 2022,a total of 571patients were eligible for this study.Among the 571 patients,505completed the study and colonoscopy,with an effective sample rate of88.4%.2.Among the 505 patients,59.0%(n=298)were male;the mean age was 48±12 years old,with the range from 18 to 82 years old.The incidence of qualified bowel preparation was 70.7%(n=357).The incidence of qualified bowel preparation of right,transverse and left colon segment were 73.3%,90.3%,and 91.5%,respectively.3.The multivariate analysis found that:age 36～45(OR=3.398,95%CI:1.270～9.093)、46～60(OR=6.204,95%CI:2.449～15.716)、>60(OR=6.766,95%CI:2.631～17.400);taking PEG as colon cleaning medicine(OR=2.924,95%CI:1.910～4.479);with the stool times of 4～5(OR=0.473,95%CI:0.259～0.865)、6～8(OR=0.275,95%CI:0.149～0.506)、≥9(OR=0.242,95%CI:0.119～0.493)were main influencing factors of bowel preparation quality.Conclusion:1.Among the 505 patients in this study,the incidence of adequate bowel preparation was 70.7%.This result is consistent with a number of previous studies,but still lower than the recommended incidence of 85%or above,which suggests the urgent need for attention on clinical practice and intervention measures to improve the quality of intestinal preparation before colonoscopy.2.The multivariate analysis found that age,kind of colon cleaning medicine,and stool frequency after taking cleaning medicine were independent factors of bowel preparation quality.Patients with older age,taking PEG as colon cleaning medicine,and having less stool frequency were more likely to have a poor quality of bowel preparation.In clinical practice,attention should be paid to optimizing the drug regimen for intestinal preparation,and personalized intervention should be carried out for high-risk groups.Thus,to better improve the quality of intestinal preparation and the success rate of colonoscopy,as well as to reduce the mortality rate of colorectal cancer.
