Clinical Study On The Effect Of Intravitreal BSS Tamponade On Lens Status And Ocular-related Parameters

Objective:To compare the changes of lens state,intraocular pressure(IOP),axial length(AL),anterior chamber depth(ACD),and corneal curvature(CC)before and after aqueous tamponade of vitreous cavity.Methods:This retrospective study included patients who underwent pars plana vitrectomy(PPV)or vitreous cavity silicone oil(SO)removal with balanced salt solution(BSS)tamponade in Jilin University,a total of 42 eyes of 21 patients(21 surgical eyes,21 control eyes).Patients were followed up for at least 6 months.According to the type of operation,they were divided into PPV group and SO-removal group.According to the type of disease,they were divided into diabetes retinopathy(DR)group and non-DR group.The patient’s age,gender,interval between preoperative and final follow-up,operation duration,best corrected visual acuity(BCVA),IOP,lens opacity,lens thickness(LT),AL,ACD,CC and other data before surgery and at the final follow-up were recorded.Results:1.At the time of the final follow-up,95.2% of the operated eyes had cataract progression or cataract surgery,which was higher than that of the control eyes(42.9%).Among them,nuclear sclerotic cataract(NSC)is the most common.2.The AL was significantly shorter(average 0.096mm),which was most obvious in the SO-removal group(average 0.118mm)(P<0.05).And the LT was significantly thicker than that before operation(average0.091mm),which was most obvious in the non-DR group(average0.125mm)(P<0.05).There were no significant changes of AL and LT between the DR group and PPV group,and no significant difference of the parameters in the control eyes and other parameters in the operated eyes at the final follow-up compared with those before operation.3.The preoperative AL,ACD,CC,and AL,ACD,and Δ’LT at the final follow-up in the DR group were smaller than those in the non-DR group,while the LT before surgery and at the final follow-up,and ΔCC were larger than those in the non-DR group(P < 0.05).There were no significant differences in other parameters between the groups.4.The preoperative ACD,CC,IOP,and ACD at the final follow-up in the PPV group were smaller than those in the SO-removal group,while the LT before surgery and at the final follow-up,and ΔCC were larger than those in the SO-removal group(P<0.05).There were no significant differences in other parameters between the groups.5.ACD was significantly negatively correlated with age,and ΔACD was significantly positively correlated with age(P < 0.05).LT was significantly positively correlated with age(P<0.05).The LT before and at the final follow-up and ΔLT were always positively correlated with the corresponding degree of nuclear sclerosis(P<0.05).Conclusions:1.Intravitreal BSS tamponade can significantly accelerate the progression of cataract,especially NSC.2.Intravitreal BSS tamponade can significantly increase LT,and the change of LT in patients without DR is significantly larger than that in patients with DR.3.LT was positively correlated with age and degree of nuclear sclerosis.4.AL becomes significantly shorter after SO-removal.5.There was a significant negative correlation between ACD and age,and a significant positive correlation between the change in ACD and age.6.ACD,CC,and IOP did not change significantly after BSS tamponade.
