Objective:To investigate the differences between desflurane and propofol in the use of intraoperative hemodynamics,wakefulness quality and incidence of adverse reactions within 24 hours after laparoscopic radical colorectal cancer resection in elderly patients.Methods:From November 2022 to January 2023,60 elderly patients(65-80 years old)who underwent laparoscopic radical colorectal cancer resection at the Third Hospital of Jilin University were selected.Group D and Group P were randomly divided,with desflurane and remifentanil used in the anesthesia maintenance process in the Group D,and propofol combined with remifentanil targeted infusion in the Group P.Both groups used the same anesthesia induction,and both maintained BIS values between 40 and 60 intraoperatively.The hemodynamic index,the total amount of remifentanil and the vasoactive drug dose,the awakening time,and the occurrence of adverse reactions within 24 hours after surgery were recorded in the two groups.Results:1.The general data(age,BMI value,ASA grade),hemodynamics(blood pressure,average arterial pressure,heart rate)before anesthesia induction(T0),surgery time and anesthesia time were not statistically significant(P>0.05)of the two groups.2.Compared with the P group,the differences in BP,MAP,and HR after endotracheal intubation(T1),pneumoperitoneum 15 min(T2),pneumoperitoneum 30 min(T3),and tracheal extubation(T4)were not significantly significant(P>0.05).In Group D,there was no significant difference between T1 and T2(P>0.05),and the remaining time periods were statistically significant(P<0.05).Compared with T2 and T3 in group P,there was no significant difference between T1 and T2 and T3(P>0.05),and the difference between the two in the remaining time period was statistically significant(P<0.05).3.The total amount of remifentanil used in group D was significantly less than that in group P,and the difference was statistically significant(P<0.05);there was no significant difference in the use of vasoactive drugs between the two groups(P>0.05).4.The spontaneous breathing recovery time(H1),eye opening time(H2),extubation time(H3),and directional recovery time(H4)of group D were significantly shorter than those of group P,and the difference was statistically significant(P<0.05).5.Two groups of patients with postoperative incidence of adverse reactions in 24 hours(such as nausea,vomiting,delirium,cognitive dysfunction,etc.)no statistical difference(P > 0.05).Conclusion:For elderly patients undergoing laparoscopic radical resection of colorectal cancer,the use of desflurane plus remifentanil for intraoperative anesthesia maintenance is more stable and has a shorter recovery time than propofol plus remifentanil;There was no statistically significant difference in the incidence of adverse reactions within 24 hours of surgery.
