| Objective:It is a promising technique to obtain lung cancer related information by ultrasound guided biopsy of suspicious lymph nodes of lung cancer patients. The main purpose of this article is to evaluate the value of biopsy of supraclavicular lymph nodes with high FDG uptake by nuclear physicians in patients with suspected lung cancer. Methods:Patients meeting the requirements can be found in our hospital database from September 2019 to March 2021.The 3-12 MHz linear ultrasonic probe(N1500,Neusoft,China)is used to guide the puncture process and ensure that the needle puncture is in the high metabolism area of PET/CT images. The diagnostic yield, malignancy rate, molecular testing results and complications were analyzed. Results:A total of 54 patients were counted, and 53 patients obtained definite pathological results through ultrasound guided tissue biopsy. The diagnostic yield was98.1%(53/54),and the malignancy rate was 96.2%(51/53).Among the 50 patients diagnosed as lung cancer through needle biopsy,38 patients were exempted from bronchoscopy,CT guided needle biopsy and diagnostic surgery.38 patients with non-small cell lung cancer(nsclc)were fully examined by molecular analysis.36.8%(14/38)patients had EGFR gene mutation,31.6%(12/38)patients had ALK gene translocation.28.9%(11/38)patients had TPS ≥ 50% for PDL-1 expression. All patients had no obvious complications, and all puncture procedures were completed within an average of 15 minutes. Conclusion:Ultrasound guided biopsy of supraclavicular lymph nodes with high FDG uptake in suspected lung cancer patients by nuclear medicine doctors can obtain a higher diagnostic result, positive rate and molecular type. And avoid more dangerous invasive examination and provide more rapid follow-up treatment. |
