Objective:To compare the anesthetic effects of 4 anesthesia methods in root canal therapy of irreversible pulpitis of mandibular posterior teeth.The 4 methods are Lidocaine block anesthesia,Articaine block anesthesia,Articaine infiltration anesthesia,Lidocaine block anesthesia combined with Articaine infiltration anesthesia.Materials and Methods:The experimental site was Shanghai Jinshan Dental Clinic.The experiment lasted from March to December 2022.The experimental subjects were 120 patients ranged in age from 18 to 60 years with irreversible pulpitis of mandibular posterior teeth.All 120 patients signed informed consent forms.The patients were randomly divided into 4 groups with 30 patients.Group A received Lidocaine block anesthesia of inferior alveolar nerve;Group B received Articaine block anesthesia of inferior alveolar nerve;Group C received Lidocaine supraperiosteal infiltration anesthesia;Group D received Lidocaine block anesthesia of inferior alveolar nerve combined with Articaine supraperiosteal infiltration anesthesia.Root canal treatment was initiated 5 minutes after the patient was anesthetized.The assistant observed the pain performance of the patient during treatment and rated a pain level with a numerical score.After the operation,the assistant immediately asked the patient about the self-evaluation of pain and rated a pain level with a numerical score.After the treatment,the assistant recorded the duration of the patient’s soft tissue anesthesia through a random telephone interview.Then,I compared the success rate,numerical rating scale score,comprehensive evaluation and duration of anesthesia of the four groups.Results:There were no statistical differences in gender composition(P=0.475),dental position composition(P=0.888)and age composition(P=0.278)among the 4 groups.The baseline data were balanced and the experimental indicators were comparable.1.The comparison between the 4 groups showed 4 results:Firstly,About the success rate,group A,Group B,Group C and group D were56.7%,70.0%,50.0% and 83.3%,respectively.The success rate of group D was significantly higher than that of group C,with statistical difference(P=0.034).Secondly,About the NRS scores,group A,B,C and D were[6.5(3,9)],[4.5(2,8.25)],[7.5(3,9)]and[2.5(0,6)],respectively.The scores of Group C were higher than that of group D,statistical differences(P = 0.005);Thirdly,About the comprehensive evaluation,group A,B,C and D were [3(2,4)],[2.5(2,4)],[3.5(2,4)] and [2(1,3)],respectively,and group A and group C were higher than group D,showing a statistical difference(P=0.011).Fourthly,about the duration of anesthesia,group A,B,C and D were[200(180,220)],[215(190,250)],[140(130,160)] and [270(250,300)],respectively.The duration of anesthesia in group D was higher than that in group A and B,and the difference between groups A and B was statistically significant(P<0.001).There was no statistical difference between group B and other groups in success rate,NRS score and comprehensive evaluation.2.The results of in-group comparison showed that there were no statistical differences in the success rate,NRS score,comprehensive evaluation and duration of anesthesia in each group among patients with gender,age and dental position.Conclusion:1.In root canal therapy of mandibular posterior teeth with of irreversible pulpitis,block anesthesia of inferior alveolar nerve combined with local supraperiosteal infiltration has the highest success rate,the longest duration and the lightest pain among the 4 methods2.In root canal therapy of mandibular posterior teeth with of irreversible pulpitis,Infiltration anesthesia is effective on mandibular posterior teeth,and the anesthesia fades quickly,which is suitable for general treatment3.In root canal therapy of mandibular posterior teeth with of irreversible pulpitis,Lidocaine and Articaine had the same curative effect.
