Objective:The objective of this study was to analyze the factors and clinical features that may lead to delayed presentation of NSTEMI patients and to investigate their impact on prognosis.In order to provide scientific basis for clinical decision-making and treatment of medical workers,improve the response ability of our hospital to NSTEMI patients,strengthen the treatment of such patients.Methods:Patients admitted to the Department of Cardiovascular Medicine,Bethune First Hospital of Jilin University from December 2020 to December 2021 with a definite diagnosis of NSTEMI were collected as follows: a total of 756 eligible patients were collected during this period,and 235 patients were excluded due to incomplete or uncertain data;A total of 521 patients were enrolled.According to the time from symptom onset to hospital visit(hereinafter referred to as the time of hospital visit),There were 124 cases in the<12hours group and 60 cases in the 12-24 hours group.Three groups of 337 patients in the>24hours group were analyzed and compared.The time of visit,age,gender,place of residence,whether patients were admitted to other hospitals,previous history,clinical symptoms,Killip grade,laboratory indicators,echocardiography,coronary angiography,number of occlusive lesions,complete revascularization,etc.,were collected.In this clinical study,Excel was used for data collection,entry and sorting,and R4.2.0statistical software was used for processing.The measurement data in line with normal distribution were expressed as mean ± standard deviation,and the non-normal distribution was expressed as median(P25,P75).The count data were expressed as percentage or rate,and the chi-square test of multiple independent samples was used for comparison between groups.The collected independent variables were classified and assigned according to the clinical significance,and all the variables were included.The variables were eliminated according to the previous literature and clinical significance and other factors.A multivariate ordered Logistic regression model was established and parallel test was conducted.All independent variables in the model were significant at the 0.05 level,and P < 0.05 was considered statistically significant.Results:1.In this study,the influencing factors and clinical characteristics of the difference in visit time of inpatients diagnosed with NSTEMI within 1 year in the Department of Cardiovascular Medicine of our hospital were studied and analyzed.The study showed that there were statistical differences among the three groups in the city of residence,symptoms of persistent chest pain,and previous PCI/CABG,P < 0.05.2.There was statistical difference between the initial myoglobin monitoring and the peak value of troponin monitoring after admission(P < 0.05).Conclusion:1.Patients living in urban areas were seen earlier after symptom onset.2.Persistent symptoms of chest pain before admission and previous history of PCI/CABG were factors that promoted patients to see the doctor earlier.
