| Objective:By analyzing the vaginal microecological status of pregnant women with gestational diabetes and its effects on maternal pregnancy outcomes and perinatal outcomes,and studying the correlation between them,this paper provides a reference for reducing adverse risks and improving maternal and infant safety.Methods:CNKI,Wanfang,Weipu,Sino Med,Pubmed,Embase,Web of Science,Cochrane Library eight Chinese and English databases were searched,and published studies on vaginal microbiome status and maternal and infant outcomes were selected in pregnant women with gestational diabetes mellitus and healthy pregnant women as experimental and control groups,and the required data were extracted after parallel quality evaluation after screening literature according to the established standards.Thirteen studies were included for meta-analysis of relevant data.Results:(1)Pregnant women with gestational diabetes were prone to vaginal microbiome imbalance(OR=2.20,95% CI [1.46,3.32],P=0.0002),and gestational diabetes mellitus was strongly associated with vulvovaginal candidiasis(OR=2.17,95% CI[1.57,3.00],P<0.00001),and Trichomonas vaginitis(OR=0.40,95% CI [0.11,1.37],P=0.14).(2)Gestational diabetes mellitus was closely associated with preterm birth(OR=2.62,95% CI [1.81,3.80],P<0.00001)and premature rupture of membranes(OR=2.36,95% CI [1.71,3.24],P<0.00001),chorioamnionitis(OR=2.14,95% CI[1.30,3.53],P=0.003),and puerperal infection(OR=2.08,95% CI [1.18,3.69],P=0.01)was associated with postpartum haemorrhage(OR=1.45,95% CI [0.85,2.47],P=0.17)and caesarean section(OR=1.16,95% CI [0.69,1.93],P=0.58).(3)Gestational diabetes mellitus was closely associated with macrosomia(OR=4.04,95% CI [2.04,8.02],P<0.0001),neonatal hypoglycemia(OR=12.56,95%CI [4.21,37.46],P<0.00001),neonatal jaundice(OR=1.80,95% CI [1.21,2.69],P=0.004),and fetal distress(OR= 3.38,95% CI [1.04,10.94],P=0.04)and neonatal infection(OR=2.86,95% CI [1.23,6.66],P=0.01)were associated with neonatal asphyxia(OR=1.58,95% CI [0.94,2.66],P=0.08),low birth weight infants(OR=0.90,95% CI [0.61,1.32],P=0.58),and fetal growth restriction(OR= 0.82,95% CI [0.36,1.86],P=0.64)had no correlation.Conclusion:Gestational diabetes is predisposed to maternal vaginal microbiome imbalance,causing vaginal microbial infection,and causing adverse maternal and perinatal outcomes. |
