Objective:1.Investigate the current situation of quality of life and internet-based nursing services for patients with urological stoma in a tertiary hospital in Jiangxi Province,and analyze the correlation between the two as well as the influencing factors of internet-based nursing demands,to provide references for the construction of internet-based hospital follow-up intervention systems and nursing interventions.2.Based on the results of previous investigations and research experiences,construct a follow-up intervention system for urological stoma patients,with the aim of providing references and guidance for the development and implementation of internet-based nursing services for this population.3.Explore the effectiveness and value of the follow-up intervention system for urological stoma patients,in order to provide theoretical and empirical references for the application and promotion of internet-based nursing services for this population.Methods:1.A convenience sampling method was used to select 183 urological stoma patients who visited a tertiary hospital’s wound and stoma outpatient clinic between October 2021 and February 2022 in Jiangxi Province.The COH-QOL-OQ,Internet-based nursing demand scale,and a self-designed general information questionnaire were used for the survey,and the Pearson correlation analysis was used to analyze the relationship between the quality of life of stoma patients and their Internet-based nursing demands.The one-factor analysis and multiple linear regression models were used to analyze the influencing factors of Internet-based nursing demands.2.Using the literature review method and group discussion,an Internet-based hospital follow-up intervention system for urological stomas was constructed.Based on the results of the literature review and current investigation,an expert group was organized to formulate the Internet-based hospital follow-up intervention system,including service content,service subjects,service items,etc.3.Using a randomized controlled trial design,118 urological stoma patients who met the inclusion and exclusion criteria were collected from a tertiary hospital in Jiangxi Province from May to November 2022,and were randomly divided into an intervention group and a control group,with 59 patients in each group.The control group received regular specialist clinic follow-up interventions,while the intervention group used the Internet-based hospital follow-up intervention system for follow-up and intervention after discharge.The patients’ stoma adaptation level,self-efficacy score,stoma-related complications occurrence rate,and follow-up satisfaction were compared before enrollment,at 6 weeks,and at 12 weeks of follow-up.Results:1.The quality of life score for urological stoma patients was 183.4±23.80,and the total score for Internet-based nursing demands was 13.24±1.66.There was a positive correlation between the two(r=0.523,P<0.001).The results of multiple stepwise linear regression analysis showed that postoperative duration,long-term place of residence,coexisting chronic diseases,self-care ability,and quality of life were all influencing factors for Internet-based nursing demands of urological stoma patients(P<0.05).2.An Internet-based hospital follow-up intervention system for urological stomas was established with clear service content,service subjects,and service items,and a follow-up pathway consisting of clinic follow-up,alliance follow-up,and Internet follow-up.3.After 6 and 12 weeks of follow-up,the incidence of complications in the platform follow-up and intervention group was lower than that in the control group(P<0.05).After 12 weeks,the stoma adaptation score,self-efficacy score,and follow-up satisfaction were significantly higher in the platform follow-up and intervention group than in the control group(P<0.05).Conclusion:1.The quality of life for home-based urological stoma patients needs to be improved.Patients with early postoperative stage,distant medical visits,multiple co-existing chronic diseases,and lower self-care ability require particular attention during the Internet-based nursing service project.Individualized follow-up guidance can be provided to enhance patients’ self-efficacy,improve their stoma adaptation ability,and enable them to return to their families and society as early as possible.2.Based on previous research and experience,an Internet-based hospital follow-up intervention system was established to provide continuity of care for urological stoma patients,providing a theoretical basis and practical implementation plan.3.The Internet-based hospital follow-up intervention system for urological stomas can help improve patients’ stoma adaptation level,self-efficacy,and follow-up satisfaction in home-based care,and contribute to controlling stoma complications,providing references for the continuity of care for urological stoma patients at home. |
