Purpose:The number of people with type 2 diabetes is on the rise and has many adverse effects on the population as well as the healthcare system in Jiangxi Province,so an accurate understanding of the type 2 diabetes disease burden can help address the many challenges it presents.The objectives of this study were to analyze the distribution characteristics of the type 2 diabetes population in Jiangxi Province,to quantitatively estimate the level of life expectancy without type 2 diabetes in Jiangxi Province and to assess the impact on life expectancy,to quantitatively estimate the disability-adjusted life year(DALY)for type 2 diabetes and the impact of different risk factors on type 2 diabetes.To provide reference and guidance for the development of targeted prevention and control management strategies,rational allocation of health resources and prioritization of reasonable interventions for type 2 diabetes in Jiangxi Province.Method:Firstly,a descriptive analysis of the prevalence of type 2 diabetes in Jiangxi Province was performed using data from the 6th National Health Services Survey of Jiangxi Province.This was followed by a combination of calculating life expectancy without type 2 diabetes and its proportion of life expectancy using the Sullivan method using the 2018 abbreviated life expectancy table for Jiangxi province.Then,the data on type 2 diabetes deaths and life expectancy by age in Jiangxi province estimated from the GBD Global Health Data Exchange database of mortality composition ratios were used to measure the year of premature death life loss(YLL),combined with the prevalence of type 2 diabetes and the proportion of various sequelae and disability weight data to measure the year of disability life loss(YLD),and by adding the two to obtain the disability adjusted life year(DALY).Finally,the risk factors for type 2 diabetes and their relative risk were selected by combining survey data and published studies,and estimate population attributable scores for each risk factor for type 2 diabetes by gender.Results:1.The prevalence of type 2 diabetes in people aged 15 years and above in Jiangxi Province in 2018 was 4.48%,and the prevalence increased from 0.15%(15 years ~group)and 0.40%(35 years ~ group)to 9.72% and 10.77%(85 years ~ group)for men and women,respectively.The prevalence rate in urban areas increased from 0.12%(15 years ~ group)to 13.33(85 years ~ group),while the prevalence rate in rural areas first increased to the highest rate of 7.51% in the 65 years ~ group,and then decreased to 3.37%(85 years ~ group).The prevalence rate was higher in urban areas than in rural areas,and the difference was statistically significant.2.Life expectancy without type 2 diabetes was 58.04 years or 96.18% of life expectancy for the male population and 61.40 years or 94.78% of life expectancy for the female population in the 15 years ~ group.Both proportions of life expectancy decreased with increasing age to 62.73 and 57.63%,respectively,by the age group of85 years.Life expectancy without type 2 diabetes was lower in men than in women in all age groups,and the proportion of life expectancy without type 2 diabetes to life expectancy was higher in men than in women.At age 15 years ~ group,the proportion of life expectancy without type 2 diabetes to life expectancy was 94.94% for urban males,93.76% for urban females,97.25% for rural males,and 95.65% for rural females,all decreasing with age,and higher for rural than urban regardless of gender.3.In 2018,the total DALYs of type 2 diabetes in people aged 15 years or older in Jiangxi province was 176,726 person-years,of which the total YLL was 66,408person-years and the total YLD amounted to 110,318 person-years,and the proportion of total YLD to total DALYs was 62.42%.the DALY rate was 477.35/100,000,the YLL rate was 179.37/100,000 and the YLD rate was 297.98/100,000.The total DALY,YLL,and YLD numbers and rates were higher in females than in males.The male DALY rate and YLL rate showed an increasing trend with age,rising from26.53/100,000 and 10.44/100,000 to 2502.20/100,000 and 1459.31/100,000,respectively.For females,it rose to reach the highest in the 75 years ~ group with2292.38/100,000 and 1009.16/100,000,respectively.The YLD rate for males and females increased from 16.09/100,000(15 years~group)and 42.92/100,000(35years~group)to 1042.89/100,000(85 years~group)and 1283.22/100,000(75years~group),respectively.4.The weighted PAF for risk factors was 33.18%(21.85-43.74)for males and27.82%(19.43-36.32)for females.For the weighted PAF of individual risk factors,the two important risk factors contributing to type 2 diabetes in both male and female populations were overweight and obesity.The lowest for males was hypertension(3.98%(2.42-5.48))and the lowest for females was smoking(0.36%(0.17-0.57)).The weighted PAF for hypertension in men and women fluctuated widely across age groups,being highest in the ≥75 years age group and lowest in the 15-44 years age group.The weighted PAF for smoking fluctuated over a wide range across age groups,being highest in the ≥75 years age group at 0.92% and lowest in the 15-44 years age group at 0.08%.Conclusion:1.The prevalence of type 2 diabetes among people aged 15 years and above in Jiangxi Province increases with age,with urban areas being higher than rural areas,and middle-aged and older people being the main group of people suffering from type2 diabetes.2.The effect of type 2 diabetes on quality of life expectancy in Jiangxi Province in 2018 increased with age and was better in the male population than in the female population and in the rural population than in the urban population.3.The DALY of type 2 diabetes in Jiangxi Province is mainly concentrated in the elderly population,and disability life loss due to type 2 diabetes is the main component of the DALY of type 2 diabetes in Jiangxi population.DALY was higher in men than in women until age 60 years ~ group.4.Nearly one-third of type 2 diabetes cases in both male and female populations can be attributed to modifiable risk factors such as being overweight or obese,low physical activity,smoking,and high blood pressure.The two main risk factors for type 2 diabetes in men and women are overweight and obesity.
