Objective:To explore the epidemic trend of influenza virus types and the clinical characteristics of inpatients in Shangrao People’s Hospital under the prevention and control of COVID-19,in order to provide new ideas and basis for the prevention and control of influenza during the COVID-19 prevention and control period. Methods:Firstly, through retrospective investigation, the monitoring results of influenza virus detection were collected from January 2020 to December 2022 in Shangrao People’s Hospital and completed by PCR technology in Shangrao CDC.Influenza virus detection positive, according to the pathogenic characteristics are divided into four groups, namely, influenza A H1N1 group, seasonal H3 group,B Victoria group and B Yamagata group.Using Excel and SPSS 27.0 software, the distribution of influenza-positive patients in each group and the general situation, clinical characteristics, treatment and outcome of hospitalized patients in each group were statistically analyzed.Results:1.From January 2020 to December 2022,a total of 4170 throat swabs were collected from influenza-like cases who were treated in Shangrao People’s Hospital;a total of 390 positive cases were detected by influenza virus PCR in Shangrao CDC,with a positive rate of 9.35%.2.Among the 390 influenza positive patients,141 cases were detected in the seasonal H3 group and 249 cases were detected in the Victoria group. There was no significant difference between seasonal H3 group and B Victoria group in outpatient and inpatient cases(P=0.172).In terms of time distribution,influenza-positive patients in 2020 were mainly B-type Victoria lineage, showing a winter peak. From the 7th week of 2020,the positive rate of influenza test began to decline, until the first week of 2021,the Victoria lineage epidemic began to appear;in 2021,influenza-positive patients were all Victoria lineage B,showing sporadic phenomenon in winter, spring and summer. In 2022,the influenza positive patients were mainly seasonal H3 group, mainly in summer, and the Victoria lineage was mainly in winter and spring. In terms of population distribution, there were statistically significant differences in the composition of different age groups(P﹤0.001).3.A total of 70 influenza inpatients were actually collected, including 31(44.29%)in the seasonal H3 group and 39(55.71%)in the B Victoria lineage group. In terms of general data, severe and critical illness(100%)was higher in seasonal H3 group;severe and critical illness(87.18%)and staff or students(69.23%)were higher in the Victoria group. The mortality rate of the seasonal H3 group was 6.45%,and the total hospitalization cost ≥ 10,000 was 25.8%,while the mortality rate of the Victoria group was 2.56%,and the total hospitalization cost≥ 10,000 was 2.56%.In terms of clinical characteristics, there were differences in gastrointestinal symptoms and complications between seasonal H3 group and B Victoria group. In terms of treatment and outcome, seasonal H3 group and B Victoria group were given anti-influenza virus treatment with good outcome.Conclusion1.The dominant strains of influenza virus have changed since the prevention and control of the new coronavirus epidemic, and there are differences in the dominant strains each year.2.In influenza-positive patients, both the seasonal H3 group and the B Victoria lineage group were mainly outpatient treatment, with fewer hospitalized patients.3.The clinical characteristics of influenza inpatients in seasonal H3 group and B Victoria group have both similarities and differences, which can help to identify the two types.4.In hospitalized patients, influenza A virus is more virulent, and the condition is relatively heavier and more expensive after infection, while influenza B virus is more likely to infect clustered populations. The prognosis of influenza-positive inpatients in the seasonal H3 group and the Victoria lineage group was good, and a few patients with underlying diseases or other pathogens had poor prognosis or even death.
