Objective:This study aims to investigate the predictive value of prognostic nutrition index(PNI)in nutritional risk assessment of elderly inpatients.Method:A total of 156 elderly inpatients in the Department of general practice and geriatrics of the First Affiliated Hospital of Nanchang University from January 2022 to November 2022 were retrospectively collected through the IIH electronic medical record system.According to the scores of the patients,they were divided into 69 patients in the normal nutrition group(MNA-SF≥12 points),42 patients in the nutrition risk group(MNA-SF 8-11 points),and 45 patients in the malnutrition group(MNA-SF <7 points).Age,sex,neutrophil count,hemoglobin,lymphocyte count,fibrinogen,etc.were compared between the three groups,and differences in PNI among the three groups of patients were analyzed.The correlation factors that might influence PNI were analyzed by Pearson correlation analysis.ROC curve and area under curve were used to analyze the predictive value of PNT nutritional risk,and consistency test was used to verify the consistency of PNI and MNA-SF in the diagnosis of nutritional risk in elderly inpatients.Results:There were no significant differences among the three groups in height,gender,smoking history,drinking history,hypertension,diabetes,PLT and CPT(P>0.05),but there were significant differences among the three groups in age,body weight,body finger,CCWBCNEU,LYM,HB,ALB,PA,TRF,RBP and PNI(P< 0.05).the mean value of PNI among the three groups was statistically significant(P<0.05).Pearmson correlation analysis found that PN was positively correlated with HB,ALB,PA,TRE and RBP and negatively correlated with age.Patients in the nutrition risk group(MNA-SF 8-11 points)and malnutrition group(MNA-SF≤7 points)were divided into one group,and patients in the normal nutrition group(MNA-SF≥12 points)were divided into the other group.The ROC curve 1 was drawn based on the MNA-SF score.The results showed that the AUC of PNT to predict the nutritional risk of patients was 0.946(95%CI 0.913-0.979),and the optimal cut-off value was 45.225.Then,patients in the malnutrition group(MNA-SF≤7 points)were set as one group,and patients in the nutritional risk group(MNA-SF 8-11 points)and the normal nutrition group(MNA-SF≥12 points)were set as the other group.Again,the ROC curve 2 was drawn based on the MNA-SF score.The results showed that the AUC of PNT to predict the nutritional risk of patients was 0.945(95%CI0.908~0.982),the optimal cut-off value was 43.45,When PNI≥45.225,it indicated that the patient was nutritionally normal.43.45<PNI<45.225,indicating nutritional risk;PNI< 43.45,indicating malnutrition.Conclusion:The incidence of nutritional risk is high in elderly hospitalized patients.PNI,a quick and simple indicator based on blood monitoring,is widely available in clinic and has certain predictive value for the assessment of nutritional risk in elderly inpatients.
