Effect Of Replacement Or Addition Of Ziprasidone On Metabolic Indices In Schizophrenia Patients With Metabolic Syndrome

Objective:Antipsychotic medication use could lead to an increased risk of metabolic syndrome(MetS)in patients with schizophrenia(SCZ).Most studies focused on aripiprazole and amisulpride,and a few studies focused on ziprasidone.Therefore,this study aimed to investigate the effects of switching to or adding ziprasidone on MetS in patients with SCZ.Methods:According to the diagnostic criteria of schizophrenia in the Edition of the《International Statistical Classification of Diseases and Related Health Problems 10th》(ICD-10)and the diagnostic criteria of MetS in the《Chinese Guidelines for the prevention and treatment of type 2 diabetes》(2017),a total of 60 patients were screened and enrolled,among whom 12 patients dropped out.Under the premise of ensuring the efficacy,28 patients were switched to ziprasidone,while 20 patients were combined with ziprasidone.Basic information of patients was collected and metabolic indexes including waist circumference,high density lipoprotein cholesterol,triglyceride,systolic and diastolic blood pressure of patients were measured and compared before and at 4 and 8 weeks after switching to ziprasidone.Results:(1)Metabolic indexes including waist circumference,high density lipoprotein cholesterol,triglyceride,systolic blood pressure and diastolic blood pressure were significantly improved after switching to ziprasidone(P < 0.05).(2)Waist circumference and systolic blood pressure decreased significantly after combined with ziprasidone treatment(P< 0.05),while there was no significant change in the other three metabolic indexes.(3)There was no significant difference in metabolic parameters between switching to ziprasidone and combination with ziprasidone.(4)Effects of different dose of ziprasidone on metabolic indexes had on significant difference.Conclusions:Switching to or combined with ziprasidone can significantly improve metabolic indexes in patients with SCZ with MetS.Ziprasidone has less impact on metabolism than other drugs including olanzapine.Switching to or combined with ziprasidone can be used as a feasible scheme for the treatment of MetS in patients with SCZ.
