| Objective:To investigate the efficacy and safety of endoscopic thyroidectomy(ET)for papillary thyroid carcinoma(PTC)through the subclavian single incision approach.Methods :A retrospective analysis was performed on 178 female PTC patients who underwent surgery at the Department of Thyroid Surgery,Hongjiaozhou,Second Affiliated Hospital of Nanchang University from December 2021 to November 2022.Among them,88 cases of transclavian single incision approach(endoscopic group)and 90 cases of traditional neck low-collar transverse arc incision(open group)were divided.The number of lymph nodes,postoperative hospital stay and near-and long-term complications of the two groups were statistically compared,and the statistical software SPASS26.0 was used to analyze,summarize and summarize.Results:All patients were successfully completed with no open transit surgery in the endoscopic group,and no permanent complications occurred in all patients after surgery.1.The age of the two groups:(33.08±8.15)years in the endoscopic group and(34.66±7.77)years in the open group,the difference was not statistically significant(P>0.05),indicating that there was no significant difference in age between the endoscopic group and the open group.2.There was no significant difference in the comparison of thyroid stimulation hormone(TSH),maximum tumor diameter,number of lesions and location between the two groups(P> 0.05)。3.There was no significant difference between the two groups during and after surgery(P>0.05),indicating that the endoscopic group could reach the same level as the open group in terms of surgical efficiency,surgical trauma and economic components.4.There was no significant difference between the two groups in the comparison of postoperative temporary laryngeal nerve(RLN)injury,superior laryngeal nerve injury(SLN),temporary parathyroid gland(PTH)hypofunction,postoperative hemorrhage and hematoma,postoperative infection,and lymphangioleak(P>0.05).5.There were obvious differences between the two groups in the comparison of neck swallowing discomfort,neck pain,numbness,pulling and compression(P<0.05),indicating that the endoscopic group was significantly better than the open group.6.There was a significant difference in postoperative paresthesias in the supraclavicular(inferior)area of the two groups(P<0.05),indicating that the open group was better than the endoscopic group.7.Incision beauty satisfaction: 76 in the endoscopic group;Open group: 20;The difference was statistically significant(P<0.05),indicating that the endoscopic group was significantly better than the open group in terms of cosmetic satisfaction with incisions.Conclusion:Transclavian single incision ET surgical treatment of PTC has the advantages of safety,efficacy and good cosmetic effect,and can obtain better clinical results. |
