Objective:Tongue squamous cell carcinoma(TSCC),abbreviated as tongue squamous cell carcinoma,is a more common oral and maxillofacial malignancy.,due to its special function and anatomical structure, the prognosis is generally poor.In order to investigate the general indicators(age, sex, blood type),pathological characteristics(tumor diameter, pathological classification, location of the primary lesion),treatment plan(radiotherapy, chemotherapy),peripheral blood indicators(neutrophils, platelets, lymphocytes, monocytes, white blood cells)and their comprehensive indicators on the recurrence of tongue squamous cell carcinoma in patients with T1-2 c N0 stage tongue, the clinical data of tongue cancer patients in the past 10 years were collected and analyzed, and the T1-2c N0 stage was constructed A nomogram prediction model of relapse-related factors in TSCC patients, and clinical validation of the model. Methods:From January 2014 to December 2022,the Department of Oral and Maxillofacial Surgery of the First Affiliated Hospital of Nanchang University performed "primary lesion resection of tongue squamous cell carcinoma" and "cervical lymph node dissection" at the same time. The collected information of 231 patients was randomly classified according to a 4:1 ratio, including 185 people in the modeling group and 46 people in the verification group. The information of patients in the modeling group was imported into SPSS25.0 for logistic regression analysis was performed to determine the risk factors of recurrence in patients with T1-2c N0 TSCC.stage, and the nomogram for predicting recurrence risk was established by R software. The consistency index(C-index)and calibration curve were used to evaluate the predictive performance and accuracy of the predictive model, and finally the decision curve analysis(DCA)was used to evaluate the clinical application value of the predictive model. Results:Among the 231 patients with stage T1-2 c N0,the number of relapse cases was76,and the recurrence rate was 33.04%.Logistic regression analysis was performed on the 17 risk factors included in the analysis, and the results were as follows: SII(immuno-inflammatory index),whether chemotherapy, tumor pathological grade and tumor maximum diameter were highly correlated with early TSCC recurrence,Therefore, these four variables are incorporated into the construction of clinical predictive model. The patient information of the validation group was used to evaluate the clinical prediction model and confirmed that the accuracy of the model was good and the clinical application value was high. Conclusion:The clinical prediction model constructed based on clinical characteristics and immune inflammation index(tumor pathological classification,SII value, whether chemotherapy and tumor diameter)can be used clinically to evaluate the postoperative recurrence probability of T1-2c N0 tongue squamous cell carcinoma patients, and provide personalized guidance for subsequent treatment of postoperative patients,in order to improve the survival time of patients with early tongue cancer.
