| Objective:Preparation of burn degree for moderate and above adult burn patients discharge preparation scale,assess patients discharge readiness,rich discharge assessment of burn patients,improve care and patients for after discharge rehabilitation care,help medical early identify lack of self management ability and other self management skills of patients,targeted health education and follow-up.Method:1.Literature analysis and interview method: nearly five years and burn patients discharge plan and discharge guidance,psychological response,rehabilitation needs related literature,in January 2022 in Nanchang a 3 armour hospital burn center purpose sampling selected 5 medical staff and 12 moderate and above adult burn patients,using semi-structured interview method combined with clinical medical opinions and patients’ real needs,analysis of clinical situation,literature analysis results combined with interview results,preliminary proposed adult moderate and above burn patients discharge readiness assessment scale entries.2.Expert letter inquiry and pre-survey: From February to April 2022,Delphi method was adopted to organize expert letters,collect expert opinions,evaluate the importance of scale items,and form the draft of discharge preparation scale for adult moderate and above burn patients;in order to further language debugging of draft scale,facilitate sampling,10 patients in burn department of a hospital in Nanchang were selected for pre-commissioning.3.Credit and validity test: From May to August 2022,182 burn patients with standard Na discharge were selected from the burn center of a Grade A hospital in Nanchang,and applied scale for investigation.SPSS22.0 conducted project analysis and exploratory factor analysis,tested reliability and validity,and formed the official version of the scale.4.Application of scale: From September to December 2022,151 eligible patients were selected from the burn center of a Grade A hospital in Nanchang to investigate the status of discharge readiness and influencing factors of adult moderate and above burn patients.Results:1.Based on the literature review and interview,a pool of items containing 4dimensions and 44 items was constructed,with physiological preparation dimension containing 13 items,psychological preparation dimension 11 items,health guidance quality dimension 13 items,and expected social relationship dimension 7 items.2.In the expert letter inquiry,32 questionnaires were issued in two rounds of expert letters,and 32 were recovered.The effective recovery rate reached 100% and the positive coefficient was 100%.The authority of the two rounds of expert letter inquiry was 0.92 and 0.91 respectively.The first draft of the scale contains 25 items in 4 dimensions.The patients did not comment on the language of the scale,so no language debugging was made.3.Four items were deleted from project analysis,exploratory factor analysis had KMO value =0.861,and four common factors were extracted.The cumulative explanatory contribution rate reached 71.455%,the total Cronbach αindex of the scale was 0.782,the Cronbachαindex of each dimension was between 0.701 and0.908,the total half reliability of the scale was 0.740,and the dimension was halved.Conclusion:The discharge readiness scale for adult moderate and above burns compiled in this study has good credit validity and can be used to evaluate patients’ discharge readiness.In the future,the sample can be expanded to apply the scale to all adult burn inpatients,to verify the reliability and validity of the scale,or to conduct longitudinal studies to analyze its dynamic changes,to improve the discharge preparation and follow-up plan,and to help patients return to social life smoothly. |
