The Relationship Between Demoralization,Adult Attachment And Coping Style In Patients With Lung Cancer

Objective: This study aims to investigate the status of demoralization,adult attachment and coping style in patients with lung cancer;analyze the influencing factors of demoralization in patients with lung cancer;analyze the correlation between demoralization,adult attachment and coping style in patients with lung cancer;and preliminarily verify whether coping style plays a mediating role in the relationship between adult attachment and demoralization,in order to provide a theoretical basis for the intervention study of demoralization in patients with lung cancer from the perspective of adult attachment and coping style.Methods: This is a cross-sectional survey study.Patients with lung cancer who were hospitalized in Shaanxi Provincial People’s Hospital from February 2022 to October2022 and met the inclusion and exclusion criteria of this study were selected as the research objects.The research tools include: general information table of patients,Demoralization Scale Mandarin Version(DS-MV),Experiences in Close Relationships Inventory(ECR)and Medical Coping Modes Questionnaire(MCMQ).Excel 2019 was used to establish a database.SPSS 22.0 software was used to carry out statistical description,independent sample t test,one-way ANOVA,Pearson correlation analysis,multiple linear regression analysis,and Process plug-in for mediating effect analysis.Results: 1.The total score of demoralization in 210 patients with lung cancer included in the study was 36.60 ± 13.89,and the scores of each dimension: emotional insecurity was 8.55 ± 3.69,depression was 8.54 ± 3.56,failure was 6.92 ± 3.14,meaninglessness was 6.53 ± 3.18,and helplessness was 6.06 ± 3.04.There were 142patients(67.6%)with moderate level of demoralization.The scores of the two dimensions of adult attachment: attachment anxiety was 66.36 ± 15.95,attachment avoidance was 53.77 ± 14.18.The scores of the three dimensions of coping style: face coping was 18.60 ± 4.09,avoidance coping was 15.32 ± 2.89,yield coping was 11.16 ±3.72.2.Correlation analysis showed that demoralization was positively correlated with attachment anxiety and attachment avoidance of adult attachment(P < 0.01).It was negatively correlated with face coping,avoidance coping(P < 0.01),and positively correlated with yield coping of coping style(P < 0.01).3.Multiple linear regression analysis showed that education level,whether combined with other diseases,attachment anxiety,attachment avoidance,face coping and yield coping were important influencing factors of demoralization in patients with lung cancer,which together explained 50.7 % of the total variation of demoralization.4.The mediating effect analysis showed that the mediating effect of yield coping,face coping between attachment anxiety and demoralization was significant,the mediating effect accounted for 33.3% and 22.3% respectively,and the direct effect accounted for 44.4%.The mediating effects of yield coping,face coping between attachment avoidance and demoralization was significant,the mediating effect accounted for 33.8% and 22.0% respectively,and the direct effect accounted for 44.2%.Yield coping and face coping play a partial mediating role between adult attachment and demoralization.Conclusions: 1.The majority of patients with lung cancer was moderate level of demoralization,accounting for 67.6%;attachment anxiety and attachment avoidance are at moderate level;they tend to adopt face coping style.2.The demoralization of patients with lung cancer was positively correlated with attachment anxiety and attachment avoidance of adult attachment.It was negatively correlated with face coping,avoidance coping,and positively correlated with yield coping of coping style.3.The main influencing factors of demoralization in patients with lung cancer:education level,whether combined with other diseases,attachment anxiety,attachment avoidance,face coping and yield coping.Patients with lower education level,combined with other diseases,choice of yield coping,with higher level of attachment anxiety and attachment avoidance had higher level of demoralization,suggesting that medical workers should pay more attention to the above patients and take targeted measures to reduce the level of demoralization.4.Coping style(yield coping and face coping)plays a partial mediating role in adult attachment and demoralization,that is,attachment anxiety and attachment avoidance of adult attachment can directly affect demoralization,and can also indirectly affect demoralization through yield coping and face coping.Medical workers can reduce the level of attachment anxiety and attachment avoidance in patients with lung cancer through corresponding measures,change negative coping style,and reduce the level of demoralization.
