ObjectiveBy reviewing domestic and foreign literature and combining with the concept of "TIME" exercise,a scientific regular exercise program was formulated for elderly maintenance hemodialysis patients,and the influence of regular exercise program under the concept of "TIME" exercise on the physical function,exercise self-efficacy and quality of life of elderly maintenance hemodialysis patients was discussed.MethodsThis study was a randomized controlled trial.Eighty-four elderly maintenance hemodialysis patients in the blood purification department of a Grade A hospital in Xi ’an were selected as the research objects,and the subjects were divided into control group and intervention group by random number table method,with 42 persons in each group.Control group received hemodialysis routine care and corresponding exercise guidance;Patients in the intervention group received regular exercise intervention under the concept of "TIME" for 3 months on the basis of routine nursing.After 3 months of intervention,the changes of physical function(STS-10,grip strength,walking speed),exercise self-efficacy,perception of exercise benefit barrier,and quality of life in the two groups were compared,and the intervention effect of regular exercise under the concept of "TIME" was analyzed.ResultsAfter 3 months of intervention,4 cases of shedding in the intervention group and 3cases in the control group,and the remaining 77 elderly hemodialysis patients completed all intervention studies.The intervention results of the two groups are as follows:(1)Comparison of baseline data of study subjects before intervention: there was no statistical significance in general data,physical function(grip strength,STS-10,walking speed),exercise self-efficacy,perception of exercise benefit impairment and quality of life of dialysis patients between the two groups before intervention(P > 0.05),indicating comparability.(2)After 3 months of intervention,the grip strength and walking speed of patients in the intervention group were significantly increased compared with before intervention,while STS-10 was significantly decreased compared with before intervention,with statistical significance(P<0.05);Compared with the control group,after 3 months of intervention,the differences of grip strength,STS-10 and walking speed between the two groups were statistically significant(P<0.05).(3)After 3 months of intervention,the score of exercise self-efficacy in the intervention group was(49.50±8.6)points,which was significantly higher than that before intervention(P<0.05),and the score of exercise self-efficacy in the control group was(43.05±10.56)points,and the difference in the level of exercise self-efficacy between the two groups was statistically significant(P<0.05).(4)After 3 months of intervention,the perceived impairment score of exercise benefit in the intervention group was significantly higher than that before intervention(69.50±7.60)points(P<0.05),and the perceived impairment score in the control group was(58.56±11.03)points.There were statistically significant differences in perceived level of exercise benefit disorder between the two groups(P<0.05).(5)After 3 months of intervention,the total score of quality of life in the intervention group was(120.68±6.17)points,and that in the control group was(111.82±8.45)points,the difference between the two groups was statistically significant(P<0.05);There were statistically significant differences in the scores of physical symptoms,fatigue and depression between the two groups(P<0.05),but no statistically significant differences in the scores of interpersonal relationship and frustration between the two groups(P > 0.05).Conclusion(1)Regular exercise helps to enhance muscle strength,improve physical function and improve quality of life in elderly maintenance hemodialysis patients.(2)Regular exercise under the concept of "TIME" can improve the exercise self-efficacy and exercise cognition of maintenance hemodialysis patients,thus enhancing the exercise compliance of patients. |
