Objective:Firstly,we aimed to clarify the research progress and frontier analysis of fabella;Secondly,to study the incidence,size and degeneration of febella in end-stage KOA(Knee osteoarthritis);Thirdly,to find the correlation of FDG(Fabella degeneration grade)with ROM(Range of motion),FFD(Fixed flexion deformity)and flexion restriction.Methods:Part 1: The Web of Science database was searched for relevant literature on fabella from January 1990 to December 2022.The included literature was analysed descriptively using Excel 2016,as well as Cite Space 6.1.R6 software for country/region,institution and author for co-occurrence analysis,journal co-citation and literature co-citation for the included literature.And cooccurrence analysis,clustering and emergent analysis for keywords.Part 2: The medical records of patients who were proposed to undergo TKA at the inpatient unit of Xi’an Honghui Hospital from June 2019 to September 2020 and the preoperative positive and lateral knee X-rays were retrospectively analysed.917 patient data were obtained,including 226 males and 691 females.917 patient data were obtained,including 226 males and 691 females.The patient’s age,length of history,gender,ROM,knee flexion angle and knee extension angle were recorded separately through the PACS system and clinical history file.In addition,the grade of osteoarthritis of the knee on the patient’s proposed side,as well as the length,thickness and grade of degeneration of the fabella were recorded in the frontal and lateral views of the knee of all patients.Statistical analysis of the data was performed using SPSS 27.0.Patient age,length of history,ROM of knee and the size of fabella were expressed as mean and standard deviation((3±S),and differences between male and female patients were tested using non-parametric tests.The chi-square test was used for the statistics of differences between the male and female proposed surgery sides.Spearman’s correlation analysis was used to analyze the correlation between FDG and ROM,FFD and knee flexion limitation.P<0.05 indicates statistical significance.Results:Part 1: A total of 106 relevant articles were obtained,and the number of articles published in fabella-related studies from 1990 to 2022 showed an overall upward trend.The country with the highest volume of publications is the United States,Imperial College London(UK)is the institution with the highest number of publications,and the author with the highest number of publications is Berthaume.The most cited journals in the literature on research in this area are CLINICAL ANATOMY and JOURNAL OF BONE AND JOINT SURGERY-AMERICAN VOLUME,and the most frequently cited is an imaging-based epidemiological survey published by Egerci OF in 2017.Its keyword-related analysis covers sesamoids,anatomy,animal models,related diseases,and surgery-related research.Part 2: Among a total of 917 patients,there was no significant difference between male and female patients in the choice of surgical side(P>0.05),but there was a significant difference in the length of history and age between male and female patients(P<0.05),and the age of female patients undergoing surgery was slightly younger than that of male patients,but the disease lasted longer.The total number of patients with fabella on the knee joint on the side of surgery was 290,and the incidence of fabella was31.62%.There was no statistically significant difference in age and ROM between male and female patients with fabella on the surgical side(P>0.05),but there was a statistically significant difference in length of history(P<0.05),and the duration of illness was higher in female patients than in male patients.In addition,the average length of the patient’s fabella was(8.04±1.37)mm,of which the average length of the female fabella was(8.06±1.39)mm,and the average length of the male fabella was(7.96±1.32)mm,and there was no statistical difference between the two(Z=﹣0.59,P>0.05),and the average thickness of the patient’s fabella was(6.02±1.12)mm,of which the average thickness of female fabella was(5.99±1.16)mm and that of male fabella was(6.11±1.03)mm,there was no statistically significant difference between the two(Z=﹣0.77,P>0.05).There was a correlation between FDG and ROM(rs=﹣0.122,P<0.05),the greater the ROM,the lighter the FDG,showing a negative correlation.there was a correlation between FDG and knee FFD(rs=0.170,P<0.05),the more severe the knee FFD,the heavier the FDG,showing a positive correlation.there was no correlation between FDG and knee flexion Statistical analysis of limitation did not correlate(rs=﹣0.077,P>0.05)Conclusion:The current research hotspots of fabella mainly focus on the epidemiology of fabella,the pain caused by fabella,and the treatment of fabella during surgery.In addition,the incidence of fabella in the knee joint on the side of surgery in patients with end-stage KOA was 31.62%.In addition,FDG correlated with ROM and FFD,with FDG showing a negative correlation with ROM and FDG showing a positive correlation with FFD,but not with the degree of knee flexion restriction.
