Objective:To evaluate the efficacy and safety of 5% lidocaine gel plaster in the treatment of acute pain of herpes zoster and its preventive effect on postherpetic neuralgia.Methods:From December 2021 to August 2022,80 inpatients with herpes zoster(in their acute phase)in the Department of Dermatology,Affiliated Central Hospital of Shenyang Medical College were included in this study.They were randomly divided into control group(40 cases)and test group(40 cases).The control group was treated with acetaminophen tramadol(Schlizzi)alone to relieve pain;In the test group,on the basis of taking the same dose of paracetamol tramadol tablets orally,5% lidocaine gel plaster(dibenium)was applied continuously to the most obvious pain area for treatment(Replace one sticker every 12 hours,2 sticker/day).The treatment of the two groups of patients was carried out according to the principle of herpes zoster treatment.Except for the different treatment schemes for pain relief,the other treatments were the same.The two groups of patients were observed at the time points before treatment(T0)and on the third(T3),the seventh(T7)and the 14th(T14)days after treatment.The VAS score was used to evaluate the patients’ pain.Before treatment(T0)and after treatment(T14),the AIS score was used to evaluate the sleep quality,the SDS score was used to evaluate the patients’ emotional state.At the same time,record the degree of improvement of patients’ self-perception and the complications and adverse drug reactions were also observed.After 3 months,all patients were followed up by telephone to see if they had postherpetic neuralgia.SPSS 25.0 statistical software was used for data analysis.The measurement data is expressed by means ± standard deviation(x ± s),the discrete variables are expressed by numbers(percentage),the multi-time point results of VAS score are compared by repeated measurement analysis of variance,the other intra-group mean before and after comparison by paired data t-test,the mean between the two groups is compared by group t-test,and the counting data is expressed by chi-square test.The difference tested will be considered statistically significant if the P value is less than or equal to 0.05.Results : All patients were treated and observed in strict accordance with the experimental plan.Before treatment,there was no significant difference between the two groups in age,sex,site of disease,VAS score,AIS score and SDS score(p>0.05);After treatment,the VAS score,AIS score and SDS score of patients in the two groups decreased compared with those before treatment(p<0.05),and the degree of decline in the test group was significantly higher than that in the control group.The difference between the two groups was statistically significant(p<0.05).No complications and obvious adverse reactions occurred in all patients.The incidence of postherpetic neuralgia in the two groups was statistically analyzed after 3 months.Only one patient in the test group had PHN,while eight patients in the control group had PHN,the incidence of PHN in the test group was significantly lower than that in the control group,and the patients who had used the plaster in the acute phase almost did not have PHN,the difference was statistically significant(p<0.05).Conclusion:5% lidocaine gel plaster is safe and effective in the treatment of acute herpes zoster.While relieving pain,it can improve the quality of sleep,relieve anxiety and depression.It has a definite curative effect and high clinical safety and it has no obvious adverse reaction.This study shows that the plaster can not only relieve the pain in the acute phase of herpes zoster,but also have a certain preventive effect on PHN,which is worthy of clinical promotion and use.
