ObjectiveChronic obstructive pulmonary diseases(COPD)are progressive and incurable,and pose a huge burden on human physical,mental,and socioeconomic health.The aim of this study is to analyze the current situation and factors influencing the quality of survival of COPD inpatients,and to investigate the mediating role of positive psychological resources(hope and optimism)between social support and the quality of survival of COPD patients.From the perspective of positive psychology,it provides a theoretical basis for improving and enhancing the survival quality of COPD inpatients and clinical psychological interventions,ultimately achieving the purpose of improving patients’ psychological health,enhancing treatment outcomes and improving survival quality.MethodsIn this study,questionnaires were administered to inpatients with chronic obstructive pulmonary disease(COPD)who met the inclusion and exclusion criteria at the Second Hospital Affiliated to Shenyang Medical College and the Central Hospital Affiliated to Shenyang Medical College in Shenyang during October 2020-2022,and the General Information Questionnaire,the Chronic Obstructive pulmonary The patients’ basic conditions,quality of survival,social support,hope and optimism were measured using the General Information Questionnaire,the Chronic Obstructive Pulmonary Disease Assessment Test(CAT),the Appreciative Social Support Scale,the Hoss Hope Scale and the Life Orientation Test Scale.315 questionnaires were distributed,310 valid questionnaires were distributed,and the questionnaire efficiency was 98.41%.Excel 2019 was used for double entry of the data,and SPSS 22.0 software was used for statistical analysis of the data and construction of the model.The analysis methods involved included:descriptive analysis of the general condition of COPD patients using frequency(N),composition ratio(%);mean ± standard deviation((?))to describe the scores of patients;independent sample t-test and one-way ANOVA to compare the differences between the quality of survival of slow-onset lung patients with different general conditions;and Pearson correlation analysis was used to assess the correlation between study variables in patients with chronic obstructive pulmonary disease;multiple linear regression was used to analyze the factors influencing survival quality;and finally,Bootstrap method was used to verify the mediating effect of hope and optimism with a significance test level of α=0.05.Results1.Total score of Hoss Hope Scale was 33.52±4.16,dimension 1 positive attitude towards reality and future was 10.96±1.62,dimension 2 positive actions taken was 10.51 ± 1.55,dimension 3 maintaining close relationships with others was 12.05±1.61;life orientation test scale was 22.81±3.38;comprehension Social support scale total score was 59.01±8.60,dimension 1 significant others support score was 16.63±5.09,dimension 2 family support score was 22.33±3.22,and dimension 3 friends support score was 20.05±3.34.2.The total CAT scale score was 21.15±6.29,and the results of stepwise regression analysis showed that the differences in CAT scale scores were statistically significant(P<0.05)among patients with different levels of dyspnea,whether they usually had chest tightness and breathlessness,participation in any form of physical exercise or sports,whether they had coughing and phlegm when they did not have a cold,and per capita monthly household income;the differences in comprehension social Support Scale score,Hoss Hope Scale score,and Life Orientation Test scale score had a significant negative effect on CAT scale score(β=-0.077,-0.091,-0.129,P<0.05).3.Pearson correlation analysis showed that the Appreciative Social Support Scale score was significantly positively correlated with the Hoss Hope Scale score and the Life Orientation Test scale score(r=0.475,0.389,P<0.01),and the Appreciative Social Support Scale score was significantly negatively correlated with the CAT scale score(r=-0.225,P<0.01);the Hoss Hope Scale score,the Life Orientation Test scale scores were significantly negatively correlated with CAT scale scores(r=-0.470,-0.396;P<0.01).4.The results of the mediation analysis showed that hope played a fully mediated role between social support and survival quality,with a mediated effect value of-0.030 and a mediated effect of 53.57%;optimism played a fully mediated role between social support and survival quality,with a mediated effect value of-0.032 and a mediated effect of 57.14%;hope and optimism played a chain mediated role between social support and survival quality,with a total mediated effect value of-0.055.Conclusions1.The higher the grade of dyspnea,the usual feeling of chest tightness,the presence of breathlessness,the absence of any form of physical exercise or sports,the presence of cough and sputum without a cold,and the lower the per capita monthly household income,the poorer the survival quality of patients.2.There were significant correlations between social support,hope,optimism and survival quality.The higher the level of social support,the better the survival quality of patients;the higher the level of hope,the better the survival quality of patients;the higher the level of optimism,the better the survival quality of patients.3.Hope fully mediated the mediating effect between social support on survival quality,optimism fully mediated the mediating effect between social support on survival quality,and hope and optimism mediated the chain effect between social support on survival quality.In clinical interventions,the quality of survival can be improved by increasing the level of hope and optimism in COPD patients.
