ObjectiveTo analyze the expression level of DDX5 in cancer tissues of patients with stage Ⅳ colorectal cancer and its correlation with clinical characteristics,and to explore the expression level of DDX5 in cancer tissues of patients with stage Ⅳ colorectal cancer and its role in prognosis evaluation.Methods Part oneA total of 230 patients with stage IV colorectal cancer who were admitted to The Central Hospital of Shaoyang from September 1st,2018 to September 30 th,2021 were included.Immunohistochemical method was used to detect the expression level of DDX5 protein in cancer tissues of patients with stage Ⅳ colorectal cancer.And the expression of DDX5 in cancer tissues of patients with stage Ⅳ colorectal cancer and its correlation with clinical characteristics were analyzed.Part twoA total of 118 patients with stage IV colorectal cancer admitted to The Central Hospital of Shaoyang from October 1st,2019 to November30 th,2020 were included.Patients were divided into low expression group and high expression group according to the expression level of DDX5 protein in cancer tissue of patients;According to the scheme recommended by the Chinese Society of Clinical Oncology(CSCO)colorectal cancer diagnosis and treatment guidelines in 2019,they were divided into chemotherapy group and chemotherapy combination targeted therapy group;Referring to the RECIST 1.1 solid tumor evaluation criteria,the objective response rate(ORR)and progression-free survival(PFS)of patients with the two treatment methods were observed.The differences in ORR and median PFS of patients with high and low DDX5 expression groups were compared to clarify the relationship between the expression level of DDX5 in cancer tissues of patients with stage IV colorectal cancer and the prognosis of patients.ResultsPart one(1)Univariate analysis showed that the high expression of DDX5 protein in cancer tissues of patients with stage IV colorectal cancer was related to the degree of differentiation and Ki-67(P<0.05);the expression of DDX5 protein in cancer tissues of patients with stage IV colorectal cancer were not related to the gender,age,tumor location,histological type,ascites,distant metastasis and mismatch repair(MMR)protein of the patients(P>0.05).(2)The results of Logistic regression analysis showed that the high expression of Ki-67 was an influencing factor for the high expression of DDX5 protein in cancer tissues of patients with stage IV colorectal cancer(r=0.234,P<0.05).Part two(1)The efficacy evaluation results showed that the ORR of the high DDX5 protein expression group in the chemotherapy group was 40.40%,and the ORR of the low expression group was 44.80%,and there was no significant difference between the two groups(P>0.05).In the chemotherapy combined with targeted therapy group,the ORR of the high DDX5 protein expression group was 63.20%,and the ORR of the low expression group was 61.50%,and there was no significant difference between the two groups(P>0.05).(2)The results of univariate analysis showed that the difference in median PFS of patients with different degrees of differentiation was statistically significant(P<0.05).The curative effect of patients with ascites group was worse than that without ascites group,and the difference was statistically significant(P<0.05).The curative effect of the patients whose MMR protein expression was d MMR group was worse than that of p MMR group,and the difference was statistically significant(P<0.05).The patients with high DDX5 protein expression level in the high expression group had poorer curative effect than in the low expression group,and the difference was statistically significant(P<0.05).There was no significant difference in the gender,age,location,histological type,Ki-67 and distant metastasis of the patients(P>0.05).(3)The multivariate analysis of Cox risk regression model showed that the risk of disease progression in the poorly differentiated group was1.901 times that of the well-differentiated group(HR: 1.901,95%CI:1.018-3.551,P≤0.05);the risk of disease progression in patients with protein status of d MMR was 2.083 times higher than that of p MMR(HR:0.480,95%CI: 0.270-0.853,P<0.05);the risk of disease progression in patients with high DDX5 protein expression was 1.623 times that of low expression group(HR: 1.623,95%CI: 1.025-2.572,P<0.05).Conclusion(1)The expression level of DDX5 protein is positively correlated with the Ki-67 positive index in cancer tissues of patients with stage IV colorectal cancer.(2)The high expression of DDX5 protein is a poor prognostic factor in patients with stage IV colorectal cancer.
