Objective:To investigate the expression and clinical significance of serum interleukin-1β、interleukin-2、interleukin-6、interleukin-8 and tumor necrosis factor-αin premature infants with necrotizing enterocolitis(NEC).Methods:A total of 44 children with NEC in the neonatal ward of Chenzhou Hospital Affiliated to Nanhua University in May 2019 to December 2021 were selected as the observation group.The observation group was divided into early NEC group and advanced NEC group according to the severity of NEC.20 non preterm infants with non digestive diseases and non infectious diseases in the same period were used as control group.Serum IL-1 β,IL-2,IL-6,IL-8 and TNF-α was detected by ELISA,and the differences of the above levels in different observation groups and between the observation group and the control group were compared.Evaluation of each index predictive scale by ROC Curve in diagnosis of NEC,and the value of combined diagnosis was analyzed by binary logistic.Results:1.The levels of serum IL-1β,IL-2,IL-6,IL-8 and TNF-α in early NEC group and advanced NEC group were significantly higher than those in the control group.And their differences were statistically significant(P < 0.05).2.The levels of serum IL-1β,IL-2,IL-6,IL-8 and TNF-α in advanced NEC group were significantly higher than those in early NEC group(P < 0.05).3.There was significant difference in the positive detection rate of serum IL-1β,IL-2,IL-6,IL-8 and TNF-α among early NEC group,late NEC group and control group(P < 0.05).The positive detection rate of early NEC group and late NEC group was significantly higher than that of control group(P < 0.05).The positive detection rate of late NEC group was higher than that of early NEC group,but there difference was no statistically significant(P > 0.05).4.The areas under ROC curve of IL-1 β,IL-2,IL-6,IL-8 and TNF-α are0.802,0.896,0.756,0.808 and 0.874 respectively.Their cut-off values for the diagnosis of NEC were 14.53pg/ml,8.25pg/ml,8.98pg/ml,27.99pg/ml and22.61pg/ml respectively.The diagnostic sensitivity was 68.18%,72.73%,75.00%,65.90% and 72.73% respectively,and the specificity was 90.00%,95.00%,60.00%,70.00% and 85.00% respectively.The joint prediction NEC model was established according to binary logistic and included in IL-1 β,IL-2 and TNF-α,The area under the ROC curve is 0.957.5.The sensitivity of combined detection of IL-1β,IL-2 and TNF-α was97.67%,the specificity was 90.48%,and the accuracy was 95.31%,which was higher than that of single diagnosis(P < 0.05).Conclusion:1.IL-1 β、 IL-2,IL-6,IL-8 and TNF-α The level was significantly increased in the early stage of neonatal necrotizing enterocolitis,which was closely related to its severity.2.IL-1 β、 IL-2 and TNF-α Combined detection has a certain clinical value in the early diagnosis of neonatal necrotizing enterocolitis. |
