| Objectives To examine the association between mean arterial pressure(MAP)and arterial stiffness in non-hypertensive subjects. Methods The Kailuan study was conducted from June 2006 to October 2007.The health providers provided following health examinations every 2 years. The subjects who completed two measurements of brachial-ankle pulse wave velocity(baPWV)and were non-hypertensive at the first baPWV measurement were observed during the second, third, fourth, fifth and sixth follow-up.The baseline data were collected during the first measurement of baPWV,and the follow-up data were collected during the last measurement of baPWV.The progression of arterial stiffness was calculated as the baPWV at the last visit during follow-up minus the baPWV at baseline.Multivariable linear regression was used to analyze the effect of baseline MAP and change of MAP during follow-up on baPWV at the end of follow-up and its progression in non-hypertensive subjects, further analyses were performed on subjects stratified by sex and age.Furthermore,we conducted sensitivity analysis by excluding the subjects who were onset of hypertension, used the treatment of hypoglycemic, used the treatment of lipid-lowering medication, and had a follow-up of less than 1 years. Results Among the 12342 subjects who met the inclusion criteria, the mean age was44.25 years old with 6844 males(55.45%).The average of follow-up time was 4.10 years, and the average progression of baPWV was 60.72 cm·s-1.In multivariable linear regression analysis, after adjustment for age, heart rate, gender, fast blood glucose and other factors,an increase of 1mm Hg in baseline MAP resulted in an increase of 9.01 cm·s-1in baPWV at the end of follow-up,4.62 cm·s-1in progression of baPWV,respectively. An increase of1mm Hg in MAP during follow-up period resulted in an increase of 6.23 cm·s-1at the end of follow up,4.04 cm·s-1in progression of baPWV,respectively.Stratified analyses by age and gender showed:both each gender and age stratified groups of subjects, baseline and MAP at the end of follow-up period were positivity associated with the follow-up baPWV and progression of baPWV.Sensitivity analysis by excluding the subjects who were onset of hypertension, used the treatment of hypoglycemic, used the treatment of lipid-lowering medication, and had a follow-up of less than 1 years also revealed similar results. Conclusions Baseline MAP and elevated MAP during follow-up are associated with arterial stiffness and its progression in non-hypertensive subjects.Figure 0;Table 8;Reference 133... |
