Objective: To investigate the relationship between antimullerian hormone and offspring birthweight in PCOS patients and to analyze the predictive value of AMH on low birth weight of offspring in PCOS patients. Methods: 1.3512 patients who underwent in vitro fertilization-embryo transfer with singleton delivery at Shengjing Hospital of China Medical University from January 2016 to October 2020 were sampled and analyzed according to inclusion and exclusion criteria, including 432 cases in the study group and 3080 cases in the control group.2.The basic information of the enrolled population was collected, including clinical information: AMH,basal sex hormones, fasting serum insulin, age, body mass index and maternal history;laboratory information: cycle type, number of embryos transferred;delivery-related information: gestational age at delivery, neonatal weight, neonatal sex, delivery method,etc.3.Group the study population according to the 25 th and 75 th percentile AMH levels,25 th and 75 th percentile offspring birth weight levels and age,and group the study group according to BMI levels.4.Statistical analysis was performed to compare the differences between AMH and offspring birth weight in the study and control groups using independent samples t-test or the Mann-Whitney Utest;Spearman correlation coefficient was used to test the possible relationship between the two variables;Logestic regression was used to discuss the effect of AMH level on low birth weight of offspring in the study group.Operating characteristics curves were prepared to show the ability of AMH to predict low birth weight in the offspring of PCOS patients. Results: 1.PCOS patients had lower offspring birth weight than controls within the normal neonatal birth weight range(P<0.001),Serum AMH levels before pregnancy in PCOS patients delivered at term were negatively associated with offspring birth weight(r=-0.618,P<0.001).2.Serum AMH levels above the 75 th percentile(9.04 ng/m L)were associated with an increased risk of low birth weight in the offspring of PCOS patients(OR: 2.177,95% CI: 1.542-3.075,P<0.001).3.Serum AMH levels predicted the area under the curve of low birth weight in offspring of PCOS patients at 0.839(95% CI: 0.800-0.878,P<0.001),with a cut-off value of 8.00 ng/m L,sensitivity of 68.40% and specificity of 80.00%.4.In this study sample,BMI levels, total testosterone levels, and fasting serum insulin levels in the study group would not have an effect on the birth weight of the offspring of patients with PCOS delivered at term. Conclusion: The serum AMH level of PCOS patients has a negative correlation with the birth weight of the offspring;high AMH level of PCOS patients is associated with the risk of low birth weight of the offspring;serum AMH level of PCOS patients has a predictive value for low birth weight of the offspring of PCOS.
