Background With the advancement of aging process,the incidence and mortality of coronary heart disease(CHD)continue to rise,which has increased the economic burden of society.Therefore,it is of great significance for patients and society to find effective methods to prevent and treat CHD.Adiponectin(APN)is closely related to development of atherosclerosis(AS)because of its anti-atherosclerotic potential.Platelet-activating factor(PAF)has pro-inflammatory effects and can expand the size of atherosclerotic plaques and affect the course of coronary artery disease.There is still disagreement about the expression of APN in CHD,and there are fewer studies related to the combined detection of APN and PAF to predict CHD.Purpose The correlation between coronary artery disease and serum APN and PAF,as well as the diagnostic value of APN combined with PAF for CHD,were examined in patients with coronary artery disease by examining the expression levels of serum adiponectin(APN)and platelet-activating factor(PAF).Methods 1.From January 2022 to June 2022,185 patients who underwent coronary angiography in the Department of Cardiology of the First Affiliated Hospital of Xinxiang Medical College due to chest pain were selected as the research subjects,including 112 males and 73 females.Patients were divided into a coronary artery disease group(115patients)and a control group(70 patients)according to the results of coronary angiography.The coronary artery disease group was subdivided into 69 cases in the stable angina group(SAP)and 46 cases in the acute coronary syndrome group(ACS)according to the examination results,or into three groups according to the number of coronary stenosis branches: single(40 cases),double(38 cases)and multiple lesions(37 cases),Gensini score was also used to assess the severity of the lesions.2.The general clinical data of the investigators were collected,including age,gender,etc;Take fasting venous blood to detect fasting blood glucose(FPG),blood lipid and other routine biochemical items,APN and PAF;Enzyme-linked immunosorbent assay was used to detect PAF,and latex immunoturbidimetric enhancement method was used to detect APN.3.The serum levels of APN and PAF were compared between groups using statistical techniques,the correlation between the APN、PAF and the degree of coronary lesions in CHD was examined,and the combined test for CHD’s diagnostic value was evaluated.Results 1.TC,LDL,FPG,CRP,SBP,DBP and PAF in coronary heart disease group were higher than those in the control group,while the levels of APN and HDL were lower than those in the control group(P<0.05).2.The levels of APN in control group>single-vessel disease group>double-vessel disease group>multi-vessel disease group,the difference was statistically significant(P<0.05);serum PAF levels in control group<single-vessel disease group<double-vessel disease group<multi-vessel disease group in the lesion group,the difference was statistically significant(P<0.05).3.The levels of APN in control group> stable angina pectoris group> acute coronary syndrome group,the difference was statistically significant(P < 0.05);serum PAF levels in control group < stable angina pectoris group < acute coronary syndrome group,the difference was statistically significant(P<0.05).4.Pearson correlation analysis showed APN correlated with FPG,TC,HDL,LDL,CRP,and SBP(r values-0.171,-0.148,0.257,-0.149,-0.451,-0.164,P< 0.05,respectively).PAF correlated with FPG,TC,LDL,CRP,and SBP(r values 0.264,,0.400,0.402,0.441,0.174,P< 0.05,respectively)There was a negative correlation between APN and PAF(r=-0.574,P<0.001).5.There is a negative correlation between APN and Gensini score(r=-0.483,P<0.001);PAF concentration was positively correlated with Gensini score(r=0.546,P<0.001).6.The factors associated with coronary heart disease selected by one-way logistic regression were FPG,TC,HDL,LDL,hypertension,SBP,DBP,APN,and PAF;then these factors were included in the multi-factor regression analysis model,and the regression equation parameters were obtained by setting coronary heart disease as the dependent variable: APN was an independent protective factor for coronary heart disease;SBP and PAF were independent risk factors for coronary heart disease.7.The results showed that the AUC of APN and PAF were 0.859 and 0.937,respectively,and the optimal threshold value of APN was 9.80 μmol/L with sensitivity of67.8% and specificity of 94.3% after calculating the Jorden index.The best threshold value for PAF was 8.48 ng/m L,with a sensitivity of 84.3% and specificity of 92.9%.Combined diagnosis was made with APN and PAF concentrations,The area under the ROC curve was 0.971,with a sensitivity of 87.8% and a specificity of 98.6%.Conclusion Serum APN was negatively correlated with the severity of coronary artery lesions in coronary artery disease,and PAF was positively correlated with the severity of coronary artery lesions in coronary artery disease.the expression rate of APN and PAF has predictive value for coronary artery disease,and the combined test of the two factors has higher diagnostic value.
