purposeAt present,many scholars take the individualized tourniquet pressure setting strategy as a research topic,but no unified understanding has been reached on the optimal strategy.In this study,the individualized pressure setting strategies of different bullying tourniquet were analyzed by using the combined use as the entry point,and thus clarified the role of different barometric tourniquet strategies in TKA patients.methodsAccording to relevant standards,a total of 120 patients were selected,all of whom underwent TKA surgery in the First Bone Joint Surgery Department of Shangqiu First People’s Hospital affiliated to Xinxiang Medical College in 2020.120 patients were randomly divided into four groups and named as groups A,B,C and D,with an average of 30 patients in each group.The tourniquet pressure was 60 k Pa;Group B received tourniquet pressure of the right upper extremity + 20 k Pa before surgery;Group C used finger-clip pulse oximeter to increase the pressure of the tourniquet,stop the inflation and record the pressure value;Group D combined group B and C with low pressure value.The TKA procedures in all the 120 patients were performed by physicians in the same treatment group.According to the patient bleeding cleaning was recorded according to the "preoperative-intraoperative-postoperative" timeline,the pain visual simulation score(VAS)and the American Special Surgery Hospital(HSS)score were performed,and the recorded data were used for statistical analysis to determine the effect.The bleeding situation of patients can be evaluated by the intraoperative bleeding amount,blood transfusion situation,hemoglobin drop amount,and postoperative flow diversion,which can be used as the basis for judging the hemostasis effect of the tourniquet.The extent of perioperative pain in KTA patients being affected by differentiated tourniquet pressure setting strategies can be judged by VAS scores in the tourniquet area and surgical area,and preoperative and postoperative HSS knee joint scores in different postoperative periods.In judging differentiation pressure tourniquet individualized pressure setting strategy for TKA patients with tourniquet area skin influence,comprehensive contrast tourniquet area swelling,skin damage,complications,such as delay rehabilitation,nerve paralysis,etc.,also include postoperative temporary use of analgesic drugs first use time and hospital stay,etc.The resultsThis study included a total of 120 patients,all of whom had undergone TKA surgery,and had detailed return visit information for each patient.The mean tourniquet pressure corresponding to groups A,B,C and D were 60 k Pa,54.57 k Pa,45.53 k Pa,35.03 k Pa,respectively.Through comparative analysis,group A and Group P <0.05 were significantly different from Group B,Group C and Group D),and group D was significantly different compared with Group B and Group C(P <0.05).The intraoperative bleeding volume,flow diversion,and postoperative hemoglobin decrease showed no significant difference(P> 0.05).A comparison of VAS scores in tourniquet areas and surgical areas in each period revealed significant differences between group A and B,C and D(P <0.05).Compared with the VAS scores of tourniquet areas and knee at 6hours after surgery,a significant difference was found between group D and B and C(P <0.05)was found.Comparing preoperative and postoperative HSS scores and postoperative hospital day showed no significant differences.Compared with the first time of temporary added analgesics,group B,C and D were all later than the control group(P <0.05),with an F value of 11.14.Comparing the difference between the peripheral thigh diameter at 6 hours and the preoperative tourniquet area,the control group was higher than the average value of groups B,C and D,with a significant difference(P <0.05)and an F value of 9.65.Regarding the transfusion treatment cases,only one case in group A and D.Regarding the cases of blisters in the tourniquet area,3,1,0,and 1 cases were reported in groups A,B,C,and D,respectively.All the four groups of patients had related complications.Regarding the case of poor healing of postoperative incision,only group A and group C.conclusionAn individualized barometric tourniquet setting strategy during TKA was studied on the premise of no interference with the surgical visual field and no increase in the amount of bleeding included in the book.It is further concluded that the combination of individualized tourniquet setting strategies during TKA can reduce the pressure value of the tourniquet.The individualized pneumatic tourniquet setting strategy can reduce patient pain,reduce the damage caused by the intraoperative application of tourniquet,and help patients recover quickly.The combination application of various personalized setting strategies has a more obvious effect.The combination of various personalized pneumatic tourniquet setting strategies for TKA patients has the characteristics of reducing pain feeling and easy operation,and has the application and promotion value in the clinical field.
