| Objective:Professor Zhang Li has a unique and profound understanding of chronic incurable wounds in previous articles and academic exchanges,as well as daily teaching and clinical work.This study aims to summarize Professor Zhang’s academic and empirical summary of the treatment of chronic refractory wounds based on the "new method of desaprophyte" through literature research and clinical retrospective research,and standardize and refine the composition and specific classification of chronic refractory wounds.The process of decay and new generation is organized and meticulous,and corresponding targeted interventions are proposed.Methods: Based on the methods of ancient literature,modern literature and clinical retrospective research,a rigorous and standardized search strategy was developed,and relevant databases were established by searching the Chinese Medical Codex,computer search databases(CNKI,Wanfang,Weipu,Pubmed),wound repair and peripheral vascular electronic medical record systems of the First Affiliated Hospital of Guangxi University of Chinese Medicine.For the literature and inpatient medical records in the database,the final included literature and inpatient medical records are determined in strict accordance with the inclusion criteria.The included research subjects sorted out the composition and specific classification of chronic intractable wounds,organized and detailed the process of decay and muscle growth,and proposed corresponding targeted interventions.Results:1.A total of 118 ancient books and 228 articles of ancient books were included in the study of ancient documents,69.49% of which recorded that the composition of chronic and intractable wounds included pus,rotting flesh and new flesh.The pus included the pus caused by fumigation of qi and blood by dampness-heat and the pus produced by hygrophytic pus.Carrion includes necrotic tissue,aging tissue and dead bone;New meat includes new granulation tissue,new epithelial tissue and transparent membrane.In the ancient books included in the study,the external treatment of chronic and refractory wounds is only a general record of treatment methods.2.A total of 94 modern literature studies were included,of which 71.28% mentioned that the composition of chronic and refractory wounds included pus,rotting flesh and new flesh.On the basis of the ancient literature,necrotic tissue fluid and residual tissue were added to the specific classification of pus and rotting flesh,and the transparent membrane forming new flesh was described in more detail.However,the understanding of the composition of chronic and refractory wounds was still insufficient.Compared with the ancient literature,the modern literature has more abundant methods for the external treatment of chronic and refractory wounds,but the process of derot and regeneration is not organized and detailed,and corresponding targeted intervention measures are proposed.3.A total of 190 clinical medical records were included in the clinical study,78.95%of which recorded the composition of chronic refractory wounds including pus,rotting flesh and new flesh.In terms of external treatment of chronic and refractory wounds,as the academic leader of wound repair and peripheral vascular Department in the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine,Professor Zhang Zhang’s team of clinicians is committed to organizing and detailing the process of derotting and regeneration,and has taken corresponding targeted intervention measures for pus,rotting flesh and new flesh.Conclusion:1.The treatment of chronic and refractory wounds in both ancient and modern literature is focused on “removing saprophyte and producing new”,but the relevant literature has not carried out a detailed and comprehensive classification of “rotten flesh”,“pus” and “new flesh” and determined the targeted external treatment program.2.The application of a group of cases in Professor Zhang’s team roughly reflects the targeted treatment of various types of “rotten flesh”,“pus” and “new flesh”,but there is a lack of relevant description on the course of disease written mainly by students.Therefore,it is meaningful to apply Professor Zhang’s detailed classification of “rotten flesh”,“pus” and “new flesh” and the targeted treatment methods in clinical and teaching. |
