Objective:Every link between the removal of pathological specimens in the operating room and the transfer to the pathology department is crucial,which can significantly affect the preservation of tissue morphology,antigen and nucleic acid.The accuracy of histopathological diagnosis is strictly dependent on adequate tissue preservation,which completely depends on the variables before analysis.The pre-treatment of surgical specimens is particularly critical,and the demand for secondary utilization of paraffin-embedded(FFPE)tissue is also increasing exponentially.It is imperative to standardize the analysis stage before specimen treatment,so as to meet clinical needs.In this study,the under-vacuum and Cooling UVSC system was used to preserve and transport surgical specimens,and the quality of specimens preserved and transported by the Sealing and cooling method was verified,and analyzed from the aspects of morphology,immunohistochemistry and quality of DNA and RNA.It also records the experience we gain in our daily practice.Method:The 200 surgical specimens preserved and transported by the under-vacuum and Cooling UVSC system were sealed and analyzed morphologically and the whole process,and 44 specimens with different Sealing and cooling types were randomly selected.After vacuum refrigeration(4°)for different time(T24,T48,T72),conventional dehydration treatment,morphology,immunohistochemistry,DNA,RNA absorbance and integrity analysis.Results:Randomly selected UVSC-stored tissues were dehydrated and evaluated by standard HE staining.From T1 to T3,the tissue samples after UVSC showed good cell morphology,well-preserved structural integrity and high protein stability.At T1 and T2,different degrees of autolysis were observed on the mucosal surface of digestive tract specimens(gallbladder and intestine)and luminal specimens(uterus and kidney)at T3(72h).No autolysis was observed at any time point in solid organs and interstitial tumors.Immunohistochemical specific staining intensity and specific antigen localization were accurate,and the expression was complete.Some T3 time points had background non-specific staining(5/44).The purity of DNA and RNA of fresh tissue and UVSC preserved tissue at 0h and 24h was within the optimal range,while the tissue at 48h and 72h was beyond the acceptable range.With the increase of vacuum storage time,the DNA bands in the region above 7 0 0 bp became weaker and degraded gradually.The mutation sites and numbers of lung cancer drver gene detected by UVSC preserved specimens(T24,T48,T72)were consistent with those of conventional treatment without refrigeration.Conclusion:The use of UVSC in the transport and preservation of fresh surgical specimens(>2cm)can well preserve the histopathology and antigenicity,as well as better quality DNA and RNA,which is suitable for morphological,antigenicity analysis and molecular pathology detection,and is conducive to standardized diagnosis.However,due to the differences between different tissue types,different standardized control procedures or refrigeration time can be used according to the type of specimens.
