| Objective: The purpose of this study is to explore the clinical efficacy of Xiaoke Pill and Wuling Powder in preventing macular edema after cataract surgery in patients with diabetes of liver and kidney yin deficiency type and cataract,based on the principle of "supplementing qi and yin,tonifying liver and kidney" and harmonizing the overall balance of yin and yang of patients,and the treatment of "promoting water and moisture,promoting blood circulation and removing stasis".Methods: Forty patients(40 eyes)of diabetes patients with cataract who had undergone cataract surgery according to the standard were selected as the study subjects,and were randomly divided into A(control group)and B(treatment group),with 20 patients in each group(n=20).The patients were randomly divided into two groups for observation according to the inclusion,exclusion and removal criteria.The control group was given tobramycin dexamethasone eye drops,levofloxacin eye drops,recombinant bovine basic fibroblast growth factor eye drops for 14 days,1-2 drops per time,4 times per day from the first day after operation.On the basis of the control group,the treatment group was treated with oral Xiaoke Pill and Wuling Powder for 14 days(7 days as a course of treatment,a total of 2 courses),and the best corrected visual acuity(BCVA),macular fovea retinal thickness(CMT),and TCM symptom scoring criteria of the patients were recorded and observed before,2 weeks after,4 weeks after and8 weeks after operation,and the incidence of macular edema in the two groups were followed up.Results: 1.In general,there is no significant difference between the treatment group and the control group in terms of gender,age,and diabetes course(P>0.05).The samples are roughly at the same baseline level,with no significant difference,which is of research significance.2.There was no significant difference between the treatment group and the control group in terms of preoperative BCVA,preoperative CMT and preoperative TCM syndrome score(p>0.05),which is of research significance.3.In terms of BCVA after operation,there was no significant difference between the treatment group and the control group in BCVA 2weeks after operation(t=0.480,P=0.637);BCVA 4 weeks after operation,the vision of the treatment group was better than that of the control group;At 8 weeks after operation,the BCVA of the treatment group was significantly higher than the average value of the control group,suggesting that there was a difference in BCVA between the treatment group and the control group at 4 weeks after operation,and the BCVA recovery of the treatment group was significantly higher than that of the control group at 8weeks after operation.4.In terms of postoperative CMT,there was no significant difference between the treatment group and the control group at 2 weeks after operation(P>0.05).The average value of CMT in the treatment group at 4weeks after operation(221.42)was significantly lower than that in the control group(244.47).The average value of CMT in the treatment group at 8 weeks after treatment(216.00)was significantly lower than that in the control group(251.79),suggesting that the therapeutic effect in the treatment group at 4 weeks and 8 weeks after operation was higher than that in the control group.5.In terms of the score of TCM symptoms after operation,the score of TCM symptoms in the treatment group was significantly lower than that in the control group at 2 weeks,4 weeks and 8 weeks after operation,suggesting that the addition and subtraction of Xiaoke Pill and Wuling Powder can effectively alleviate the TCM symptoms of the experimental subjects.6.In terms of the total effective rate of western medicine,the total effective rate of the treatment group was 94.73%,and the total effective rate of the control group was 68.42%.It can be seen that the curative effect of the treatment group is more obvious.7.In terms of the total effective rate of traditional Chinese medicine,the total effective rate of the treatment group was 89.47%,and the total effective rate of the control group was 15.79%.It can be seen that the improvement of systemic symptoms in the treatment group was more effective than that in the control group.8.The incidence of macular edema in the two groups: 31.58% in the control group and 5.26% in the treatment group.The incidence rate of macular edema in the treatment group was significantly lower than that in the control group.9.In this study,there was a total of 1 patient with elevated intraocular pressure,and the other patients did not have high intraocular pressure and ocular inflammatory reaction.Conclusion: Xiaoke pill combined with Wuling powder has a certain effect on preventing macular edema in patients with diabetes of liver kidney yin deficiency type after cataract surgery,and can improve the patients’ vision and TCM symptoms;Chinese medicine has good safety and feasibility in treating this disease. |
