| Objective:LDH is a common disease in spinal surgery.The incidence rate is increasing year by year.In terms of the treatment of LDH,the Wechsler bone-setting manipulation created by Wei Guikang,a master of traditional Chinese medicine,is a method for treating spinal soft tissue injury and spine related diseases summarized in the course of long-term clinical practice.The Wechsler bone-setting manipulation is characterized by taking the basic theory of traditional Chinese medicine as the guide,combining modern anatomy,biomechanics principles,etc.,and has certain characteristics and advantages.LDH has always been one of the key research contents of Wei Lao.In order to further explore the evaluation of the curative effect of Wechsler’s osteosetting manipulation on LDH patients and its impact on the lumbar-pelvis parameters,this study adopted a randomized controlled trial to explore the advantages of Wechsler’s osteosetting manipulation in the treatment of LDH,and to study its changes in the mechanical balance parameters of lumbar-pelvis,so as to provide better evidence for the clinical promotion of Wechsler’s osteosetting manipulation in the treatment of LDH.Methods:In this study,60 patients who met the diagnostic criteria and inclusion criteria of LDH were randomly divided into observation group and control group,with 30 patients in each group.A randomized controlled study method was used.The observation group was treated with Wechsler’s osteosetting manipulation,while the control group was treated with conventional osteosetting manipulation.All patients in each group were treated once every other day,25-30 minutes each time,seven times for a course of treatment,a total of 13 days.The subjects were asked to complete the filling of personal information,JOA score,VAS score,ODI score and other information,as well as the collection of clinical symptoms,physical signs and other medical history,and to collect the lumbar anterolateral radiographs and pelvic radiographs of the subjects before and after treatment,and at the same time record and calculate the values of lumbar-pelvic imaging parameters(sacral tilt angle,lumbar lordosis angle,lumbar Cobb angle,pelvic tilt angle)before and after treatment.All data were processed using SPSS 20.0 statistical software.Results:(1)At the end of this study,a total of 60 patients were included.During the period,3 patients fell off and 57 patients actually completed,including 29 patients in the observation group and 28 patients in the control group.(2)Baseline conditions of the two groups: gender,age,course of disease,JOA score,VAS score and ODI score,sacral inclination angle,lumbar lordosis angle,lumbar Cobb angle and pelvic inclination angle had no significant difference before treatment(P>0.05),which was comparable.(3)Intra-group comparison: After treatment,the values of sacral tilt angle(SS)and lumbar lordosis angle(LL)were increased,and the values of lumbar Cobb angle and pelvic tilt angle were decreased in both groups compared with those before treatment.The difference in angle changes was statistically significant(P<0.05);Both groups can reduce VAS score and ODI score,and improve JOA score,the difference was statistically significant(P<0.05).(4)Comparison between groups: the total effective rate of the observation group was 93.11%,and that of the control group was78.58%.After treatment,the improvement of SS,LL,lumbar Cobb angle and pelvic tilt angle in the observation group was significantly better than that in the control group(P<0.05).After treatment,the JOA score in the observation group increased more than that in the control group,and the VAS score and ODI score decreased more than that in the control group.The difference between the two groups was statistically significant(P<0.05).(5)Both groups was no obvious adverse reaction.The safety rating was 1.Conclusion :(1)Both Wechsler’s osteotomy and conventional osteotomy are effective for LDH,and Wechsler’s osteotomy is superior to conventional osteotomy.(2)Both Wechsler’s osteopathic manipulation and conventional osteopathic manipulation can improve the lumbar-pelvis related parameters of LDH patients,and the effect of Wechsler’s osteopathic manipulation on improving the lumbar-pelvis parameters of LDH patients is better than that of traditional manipulation.Both of the two manipulations have certain clinical significance for remodeling the lumbar-pelvis biomechanical balance.(3)Wechsler’s bone setting manipulation and conventional bone setting manipulation can improve the pain symptoms and daily activity functions of such LDH patients,and the clinical effect is accurate.(4)Wechsler osteotomy and conventional osteotomy have no obvious adverse reactions,are safe and reliable,and are worthy of clinical promotion. |
