| Objective:1 Through the study of ancient literature and modern literature,this paper summarizes the concepts and methods of treatment of chronic refractory wounds in ancient and modern traditional Chinese medicine,and discusses the rules and limitations of diagnosis and treatment of chronic refractory wounds.to provide theoretical support for the academic concept of professor Zhang Li in the treatment of chronic refractory wounds.2 Through a retrospective study on the case data of chronic refractory wounds diagnosed and treated by Professor Zhang Li’s team,this paper combs and summarizes Professor Zhang Li’s academic point of view and clinical experience in the treatment of chronic refractory wounds.Methods:1 Ancient literature research: set the search words,take the fifth edition of Chinese Medical Code as the retrieval platform,search the TCM disease names related to chronic refractory wounds,screen the relevant ancient books and articles about the etiology,treatment principles and treatment of chronic refractory wounds according to the inclusion and exclusion criteria,input the included provisions information into the Excel software,and establish a database.Summarize the understanding and experience of ancient physicians on the etiology,treatment principles and related treatment of chronic refractory wounds.2 Modern literature research: set key words,take China knowledge Network(CNKI),Wanfang data platform(WANFANG),VIP Chinese Sci-tech Journals Database(VIP)as the retrieval platform,through the professional retrieval methods in the three databases,search the clinical observation literature about the treatment of chronic refractory wounds by traditional Chinese medicine in the past 10 years.Select the literature according to the inclusion and exclusion criteria,extract the basic information included in the literature,study the name of the disease,the number of cases,relevant modern scientific and technological means,treatment methods and other information,input the information into Excel software,establish a database,and summarize the clinical status of modern traditional Chinese medicine in the treatment of chronic refractory wounds.3 Case retrospective study: the case data of patients with chronic refractory wounds treated by Professor Zhang Li’s team were collected from January 1,2018 to December 31,2021,who were hospitalized in the Department of Peripheral Blood vessels and in the first affiliated Hospital of Guangxi University of traditional Chinese Medicine.The cases were screened according to the inclusion and exclusion criteria,and the basic information,wound types,factors affecting wound healing,relevant modern scientific and technological means,treatment methods,wound outcome and other information were extracted and summarized,and the data were statistically analyzed.On this basis,Professor Zhang Li’s academic point of view and clinical experience in the treatment of chronic refractory wounds were combed and summarized.Results:1 The study of ancient literature has included a total of 236 articles of ancient books,belonging to 107 ancient books.According to the study of ancient literature,it is concluded that the causes of chronic refractory wounds in ancient Chinese medicine are mainly "deficiency,damp-heat,heat,poison,blood stasis and phlegm",which is basically consistent with the viewpoint of "blood stasis,deficiency,evil and decay" in modern Chinese medicine.Only four articles recorded that chronic refractory wounds were related to thirst.Ancient Chinese medicine was restricted by the development of medical technology of the times,and the understanding of the primary disease of chronic refractory wounds was limited.it has not been fully realized that most chronic refractory wounds are developed on the basis of primary diseases,rarely for the treatment of chronic refractory wounds,and the treatment of chronic refractory wounds is mainly "removing saprophytic muscle".2 Modern literature research included a total of 121 articles of clinical observation of traditional Chinese medicine in the treatment of chronic refractory wounds in the past 10 years,including 11111 cases.Compared with ancient Chinese medicine,modern Chinese medicine has a further understanding of the primary disease of chronic refractory wounds,generally recognizing that most of chronic refractory wounds develop on the basis of primary diseases,and attach importance to the treatment of primary wounds.Modern Chinese medicine and ancient Chinese medicine pay attention to the treatment of chronic refractory wounds with the same concept,but it is aimed at wound treatment on the basis of the treatment of primary disease.A small number of modern Chinese medicine treat the factors affecting wound healing,but generally do not pay enough attention to the factors affecting wound healing.And the importance of modern Chinese medicine to the application of modern science and technology in the diagnosis and treatment of chronic refractory wounds needs to be improved.3 A total of 357 cases were included in the case retrospective study,which reflects Professor Zhang Li’s academic concept of treating chronic refractory wounds.This case retrospective study summarizes that Professor Zhang Li’s team attaches importance to the application of relevant modern scientific and technological means in the diagnosis and treatment of chronic refractory wounds,and carries out targeted treatment to grasp the outcome and prognosis of the wound as a whole after identifying the disease,primary disease and factors affecting wound healing according to the four diagnoses and with the help of necessary modern scientific and technological means.According to the primary disease,wound condition,physical condition,personal demands and hospital conditions,we should adopt the combination of holistic treatment and local treatment,and if necessary,choose the treatment plan reasonably and achieve good curative effect.Conclusion(s):1 In ancient Chinese medicine,the understanding of the primary disease of chronic refractory wounds is very limited,there is little treatment for the primary disease of chronic refractory wounds,and the treatment is mainly based on "removing saprophytic muscle".Modern traditional Chinese medicine attaches importance to the treatment of chronic refractory wound primary disease,but there are few treatments aimed at the factors affecting wound healing.and in the process of diagnosis and treatment of chronic refractory wound primary disease and factors affecting wound healing,the degree of attention to the application of modern science and technology needs to be improved.2 Based on the diagnosis and treatment rules and limitations of ancient and modern traditional Chinese medicine for chronic refractory wounds,combined with years of experience in understanding the theory of typhoid and miscellaneous diseases and clinical diagnosis and treatment experience,professor Zhang Li,a tutor,puts forward the academic idea of seeking causes for the treatment of chronic refractory wounds,and has achieved good results in the diagnosis and treatment of chronic refractory wounds,which is worth popularizing in clinical practice.3 The key to the treatment of chronic refractory wounds lies in:(1)first of all,the disease,primary disease and factors affecting wound healing should be identified.(2)the identification of chronic refractory wound disease,primary disease and factors affecting wound healing must be combined with modern scientific and technological means.(3)the diagnosis and treatment of chronic refractory wounds need to be evaluated in the whole process,and it is decided to continue or transfer to other methods according to the evaluation results,dynamic observation and treatment.(4)choose the treatment plan reasonably according to the primary disease,wound condition,physical condition,patient demand and hospital condition. |
