Accurate Sampling Method Of Prostate Cancer Tissues With 3D Stereo Location

Objective:To design a 3D stereotaxic sampling method to accurately and quickly obtain prostate tumor tissue from the isolated prostate gland after surgery in a short time.Methods:This study collected the clinical data of 111 patients diagnosed with prostate cancer by perineal prostate biopsy and underwent laparoscopic radical prostatectomy(LRP)from November 2020 to April 2022.Preoperative multi-parameter magnetic resonance imaging(mpMRI)data of all patients were interpreted by a senior radiologist.The prostate cancer specimens of 57 patients were collected by cognitive sampling method,and the prostate cancer specimens of the remaining 54 patients were collected by 3D stereolocalization method.The influencing factors of the two sampling methods were divided into negative group and positive group.Chi-square test was used to compare the positive rate of the two sampling methods,and correlation test was used to analyze the influencing factors of the two sampling methods.The optimal cut-off value of tumor volume when the negative predictive value(NPV)of the two sampling methods was 100%was taken as the critical value of negative sampling.The range of tumor volume collected by the two methods was comparedResults:There were 57 cases of cognitive sampling and 54 cases of 3D stereoscopic sampling.There was no significant difference in basic data between the two groups.The positive rate of cognitive sampling(17/57,29.82%)and 3D stereopositioning sampling(33/54,61.11%)was significantly higher than that of cognitive sampling(P=0.001).In terms of cognitive sampling,tumor volume(P<0.001),number of puncture positive cores(P=0.04),prostate imaging Report and Data System(PI-RADS)score(P<0.001),and clinical T stage(P=0.0029)were related to the positive rate of sampling.In 3D stereoscopic sampling,tumor volume,number of positive needles,PI-RADS score and clinical T stage were not significantly correlated with the sampling positive rate,with P values of 0.68,0.45,0.78 and 0.23,respectively.When the negative predictive value(NPV)of the two methods was 100%,the optimal cut-off value of tumor volume was taken as the critical value of negative sampling.When the tumor volume of cognitive sampling was ≤0.4 1cm3,all samples could be considered negative;when the tumor volume of 3D sampling was ≤0.26cm3,all samples could be considered negativeConclusion:In this study,3D stereotactic sampling of prostate cancer tissue was adopted,which significantly improved the positive sampling rate of prostate cancer tissue specimens,and made the positive sampling rate not affected by tumor volume,number of positive prostate puncture needles,PI-RADS score,and clinical T stage.Moreover,the volume range of 3D sampling was larger than that of cognitive sampling.
