Efficacy Of External Treatment With Traditional Chinese Medicine For Pressure Sore:A Meta-Analysis

Objective:Usingvia the method of meta-analysis,we evaluated the clinical randomized controlled trials of external treatment of pressure sore with traditional Chinese medicine,so as to provide the diagnosis and treatment ideas and reference for the treatment of pressure sore in the future.Methods:According to the established retrieval strategy,literature retrieval was carried out in seven databases(Wanfang,CNKI,Vip,Sinomed,Pubmed,embase,cochranelibrary)to search the controlled studies on external treatment with traditional Chinese medicine for pressure sore,which published from December 2012 to December 2022.All the studies were screened according to the inclusion and exclusion criteria,and the quality of the researches was evaluated,the data were collated,and charts were drawn.Revman 5.4 was used to analyze the data,and then the results were obtained.Results:After screened,29 trials in Chinese and 1 trial in English were included in this study,with a total sample size of 2568 cases;All included articles mainly evaluated the clinical efficacy of external treatment with traditional Chinese medicine for pressure sore in terms of wound healing time,wound healing rate,total effective rate and granulation tissue growth time.The results are as follows:(1)Wound healing time:There are 10 articles that record the wound healing time,and the difference is statistically significant[OR=-0.65,95%CI(-0.82,-0.48),Z=7.59,P<0.01],which shows that the wound healing time of the traditional Chinese medicine treatment group is shorter than that of the western medicine group.Further subgroup analysis shows that the wound healing time in the external treatment group with traditional Chinese medicine was shorter than that in the western medicine group no matter of the course of treatment[OR=-8.70,95%CI(-9.21,-8.20),Z=33.84,P<0.01].(2)Cure rate:There are 22 trials recorded the wound healing rate,with good homogeneity and stable outcome indicators,which fit to fixed effect model(P=0.46,I~2=0%).And the difference was statistically significant,indicating that the cure rate of the external treatment group with traditional Chinese medicine was higher than that in the control group[OR=3.49,95%CI(2.81,4.33),Z=11.35,P<0.01].(3)Total efficiency:There are 24 studies reported the total effective rate,and the difference is statistically significant,which shows that the total effective rate of the external treatment group with traditional Chinese medicine is significantly higher than that of the western medicine group[OR=6.53,95%CI(4.40,9.67),Z=9.35,P<0.01].In subgroups analysis according to the length of treatment,it is shown that the total effective rate of external treatment with traditional Chinese medicine for pressure sore is higher than that of western medicine group no matter of the duration of treatment[OR=7.83,95%CI(I5.75,10.66),Z=13.05,P<0.01].(4)Time required for granulation growth:There are 4 trials recorded the time required for granulation growth,and the difference is statistically significant[OR=-2.61,95%CI(-4.20,-1.02),Z=3.22,P<0.01].It is shown that the external treatment with traditional Chinese medicine is more effective in speeding granulation growth compared with the western medicine group.(5)Safety:There are 10 papers mentioned adverse reactions,and the other papers did not record occurrence of adverse reactions and unsafety events,it can be inferred that the external treatment with Chinese medicine for pressure sore has certain safety.In the future,higher evidence level trials are still needed to prove the safety of external treatment with traditional Chinese medicine for pressure sore.Conclusion:Through database retrieval and data analysis,it is shown that external treatment with traditional Chinese medicine can significantly shorten the healing time of pressure sore wounds,enhance the cure rate,increase the total effective rate,speed the granulation growth,and effectively alleviate the disease compared with western medicine treatment,then obtain a more satisfactory clinical effect.
