| Objectives:Understand the blood pressure control status of elderly hypertensive patients aged 65 and above in Cangxian,Cangzhou City,and analyze the influencing factors of blood pressure control,providing reference for improving the health management effectiveness of elderly hypertensive patients based on national basic public health services..Method:Select hypertensive patients aged 65 and above who have been included in basic public health service health management for at least one year in Cangxian County,Cangzhou City,Hebei Province as the research subjects to understand the effectiveness and influencing factors of health management.By utilizing the contracted services of family doctors,after a management cycle,the effectiveness of hypertension control in both contracted and non contracted populations was observed and logistic regression analysis was conducted.Result:Among elderly hypertensive patients aged 65 and above,the control rate of hypertension is higher in females than in males;Younger than older;Having a partner is higher than having no partner;Those without family history of hypertension were higher than those with family history of hypertension;The payment methods for medical expenses for urban and rural residents and others are higher than those for urban employees,with statistical differences(P<0.05).Patients who smoke,drink alcohol,and do not exercise have a low blood pressure control rate(P<0.05);Low body mass index,good adherence to medication,high adherence to medication,more follow-up visits,longer management years,and higher blood pressure control rate(P<0.05).After implementing the management of family doctor contract services,the blood pressure control compliance rate of contracted patients significantly increased compared to before(87.7%vs 50.6%),and was also higher than that of the non contracted population(87.7%vs 66.1%),with statistical differences(P<0.05).After strengthening the management of contracted patients,there was a significant improvement in their body mass index,exercise,compliance behavior,and medication adherence compared to before(P<0.05).The results of Logistic regression analysis showed that the probability of blood pressure control was higher in patients with other modes of payment than in urban workers(OR=13.770,95%CI:3.794-50.5717,P=0.000)The probability of blood pressure control was higher in urban and rural residents than in urban workers(OR=5.796,95%CI:1.807-18.5964,P=0.003)The rate of blood pressure control was 1.808 times higher in nonsmokers than in smokers(OR=1.808,95%CI:1.122-2.916,P=0.015),the rate of blood pressure control was 12.892 times higher in patients with hypertension who did not drink(OR=12.889,95%CI:7.429-22.362,P=0.000),hypertension patients who exercised were 3.628 times more likely to have blood pressure control than those who did not exercise(OR=3.630,95%CI:1.767-7.448,P=0.000),the rate of blood pressure control was 4.853 times higher in patients with contracted hypertension than in patients without contracted hypertension(OR=4.851,95%CI:3.126-7.534,P=0.000).The mode of payment of medical expenses,smoking,drinking,exercise and the contracted management service of family doctor were the influencing factors of hypertension control rate.Conclusion:The payment method of medical expenses,smoking,drinking,exercising,and the management of family doctor contract services are the influencing factors of blood pressure control rate in elderly hypertensive patients aged 65 and above.By implementing family doctor contract service management,it is possible to improve their body mass index,exercise,compliance behavior,and medication compliance,thereby increasing the blood pressure control compliance rate of elderly hypertensive patients. |
