Objective: To observe and study the clinical efficacy of radiofrequency ablation combined with ozone in the treatment of low back pain(LBP)patients with Modic modification,in order to provide a more appropriate,safe,efficient and doctor-patient satisfactory treatment for clinical diagnosis and treatment of low back pain patients with Modic modification.Method: Clinical data of patients with low back pain who were admitted to the orthopedics department of our hospital from October 2020 to June 2022 and were indicated by MRI for Modic changes were retrospectively analyzed.Sixty-eight patients with low back pain belonging to modic type I were selected.According to different treatment regiments,the patients were divided into control group(oral cincloxib capsules,traditional Chinese medicine coating and traditional Chinese medicine ironing treatment)and observation group(radiofrequency ablation combined with ozone surgery).35 patients in the control group,including 20 males and 15 females,aged 40-69 years old,mean(51.34±7.72)years old.Course of disease was(27.43±12.59)months,mean(3.4±1.3)years;There were 6 cases of L3/4,12 cases of L4/5 and 17 cases of L5/S1.Another 33 patients were included in the observation group,including 21 males and 12 females.The average age was(50.76±6.72)years.The course of disease was(24.94±11.95)months,with an average of(2.2±1.2)years.There were 7 cases of L3/4,12 cases of L4/5 and 14 cases of L5/S1.VAS scores and ODI scores of the two groups were compared and analyzed 1 day before treatment,1 day,1 week,1 month,and 3 months after treatment.Modified Mac Nad standard efficacy evaluation method was adopted during terminal follow-up.SPSS 26.0 statistical software was used for data analysis and processing in this study.All measurement data were expressed as mean ± standard deviation(X±S).If the data were in line with normal distribution,T-test and repeated measurement model were used to test.If the distribution does not conform to normal,the non-parametric test and correlation test are used.Statistical results If P < 0.05 is statistically significant,otherwise,the difference is not statistically significant.Results: A total of 68 patients were included and collected in this study.All patients successfully completed the treatment regimen according to the course of treatment,without serious adverse events or death cases.1、General information: The enrolled patients were randomly divided into 2 groups(observation group and control group),with 33 cases in the observation group and35 cases in the control group.There were no significant differences in gender,age,course of disease and lesion distribution segments between the two groups(P >0.05),which was not statistically significant and comparable.2、VAS score: There was no significant difference in VAS scores between the two groups 1 day before treatment(P > 0.05).The VAS scores of the two groups showed a decreasing trend with the passage of time points after treatment,and the changes of VAS scores at each time point were statistically significant compared with those before treatment(P < 0.05).The VAS scores of observation group were lower than those of control group at 1 day after treatment,1 week after treatment,1month after treatment and 3 months after treatment,and the differences were statistically significant(P < 0.05),while there was no significant difference in VAS scores of control group at 1 day after treatment compared with before treatment(P > 0.05).There were no significant differences in VAS scores between the two groups at 1 month and 3 months after treatment(P > 0.05).3、ODI score: There was no significant difference in ODI scores between the two groups 1 day before treatment(P > 0.05).The ODI score of the two groups showed a downward trend with the passage of time points after treatment,and the change of ODI score at each time point was statistically significant compared with that before treatment(P < 0.05).ODI scores in observation group were lower than those in control group at 1 day,1 week,1 month and 3 months after treatment,and the differences were statistically significant(P < 0.05).4、Efficacy evaluation of modified Mac Nab: In this study,at the last follow-up 3months after treatment,the modified Mac Nab standard efficacy evaluation method was used to determine the therapeutic effect.The overall excellent and good rate of the observation group was 93.9%,a total of 31 cases,and that of the control group was 80.0%,a total of 28 cases.Chi-square test was performed on the number of cases in the distribution of curative effect between the two groups,and the result was P < 0.05,indicating that the difference in the excellent and good rate between the observation group and the control group was statistically significant.The excellent and good rate in the observation group was higher than that in the control group,and the proportion of excellent cases in the observation group was significantly higher than that in the control group.Conclusions: 1 、 Compared with oral crecoxib capsules,traditional Chinese medicine rub and traditional Chinese medicine ironing,radiofrequency ablation combined with ozone can effectively reduce patients’ pain in the treatment of Modic patients with low back pain,and will not cause adverse effects on their daily work and life.2、Radiofrequency ablation combined with ozone therapy has higher clinical application value than oral crecoxib capsules,traditional Chinese medicine coating and traditional Chinese medicine ironing in terms of the speed of onset time,superior rate of curative effect and stability of curative effect.
