Objective:To investigate the correlation between the levels of Mesencephalic Astrocytederived Neurotrophic Factor(MANF)and Leptin in the midbrain and other clinical biochemical and metabolic indicators,and further discuss the correlation between MANF and leptin and type 2 diabetes mellitus(T2DM)and obesity.Methods:36 patients with T2 DM who were hospitalized in the Department of Endocrinology of Northern Jiangsu People’s Hospita from September 2021 to December 2022 and 36 healthy subjects who were hospitalized in the physical examination center of Northern Jiangsu People’s Hospita from September 2021 to December 2022 were included.The two groups were divided into normal weight subgroup(NW)and overweight/obese subgroup(OW/OB)according to body mass index(BMI),18.5kg/m~2< BMI<25kg/m~2 or BMI≥25kg/m~2.The subjects’ general data were recorded.Fasting serum MANF and leptin levels were determined by enzyme-linked immunosorbent assay(ELISA),and fasting blood glucose,glycosylated hemoglobin,TG,TC,HDL-C,LDL-C and other biochemical indexes were determined.The statistical software SPSS 26.0 was used to compare the changes of MANF and leptin levels in people with different BMI,and to explore their correlation with T2 DM and obesity.Results:1.Comparison between OW/OB-DM group and NW-DM group: BMI,FPG,TG,Fins and HOMA-IR of OW/OB-DM group were higher than those of NW-DM group(P<0.05).MANF and leptin levels in OW/OB-DM group were higher than those in NWDM group(p <0.05).2.Comparison between OW/OB-DM group and OW/OB-NGR group: FPG,HbA1 c and HDL-C in OW/OB-DM group were higher than those in OW/OB-NGR group,but HDL-C was lower than that in OW/OB-NGR group(p<0.05);The leptin level in OW/OBDM group was higher than that in OW/OB-NGR group,but the MANF level was lower than that in OW/OB-NGR group(p <0.05).3.Compared with NGR group,FPG and HbA1 c in T2 DM group were higher than those in NGR group,while HDL-C level in T2 DM group was lower than that in NGR group(p <0.05).Leptin level in T2 DM group was higher than NGR group,but MANF level was lower than NGR group(p <0.05).4.Binary logistic regression analysis showed that MANF and leptin were independent risk factors for overweight/obesity in normal-weight T2 DM patients,and the AUC of overweight/obesity in normal-weight T2 DM patients was 0.713.The AUC of overweight/obesity in T2 DM patients with leptin alone was 0.705,and the AUC of leptin combined with leptin was 0.793,which was better than that of leptin alone.Leptin was an independent risk factor for T2 DM in patients with simple overweight/obesity,and the AUC of leptin for T2 DM was 0.821.MANF and leptin are independent risk factors for T2 DM in healthy people,and the combined effect of MANF and leptin on T2 DM in healthy people is better than that of leptin alone.5.Correlation analysis showed that MANF level was negatively correlated with FPG and HbA1 c,and positively correlated with BMI,TC,LDL-C and leptin(p <0.05).Leptin was positively correlated with BMI and HbA1 c.(p <0.05).Conclusion:1.MANF level in OW/OB-DM group was higher than that in NW-DM group,but there was no difference between OW/OB-DM and OW/OB-NGR groups,suggesting that MANF is an independent risk factor for predicting overweight/obesity in normal weight type 2 diabetes patients,and has diagnostic value for predicting overweight/obesity in normal weight type 2 diabetes patients.2.The level of MANF in NGR group is higher than that in T2 DM group,suggesting that MANF is related to the occurrence and development of T2 DM in healthy people,and MANF is an independent risk factor for predicting the occurrence of T2 DM in healthy people,which has diagnostic value for predicting the occurrence of T2 DM in healthy people.3.The leptin level in OW/OB-DM group was higher than that in OW/OB-NGR group,and the leptin level in T2 DM group was higher than that in NGR group,suggesting that leptin is related to the occurrence and development of T2 DM,which has diagnostic value for predicting the occurrence of T2 DM in patients with simple overweight/obesity and healthy people.4.The combined prediction effect of MANF and leptin on the occurrence of overweight/obesity in normal weight T2 DM patients and T2 DM in healthy people was better than the two single judgments.5.MANF level was negatively correlated with FPG and HbA1 c,and positively correlated with BMI.It is suggested that MANF level can be affected by weight control and blood glucose adjustment,which may delay the occurrence and development of T2 DM and obesity.MANF may provide a new prospect for the prevention and early diagnosis of T2 DM and obesity.
