Background: With the development of society and medical level,the requirements for the nursing level of nurses are becoming stricter and stricter,and the demand for nurses is growing.The intensive care unit(ICU)receives a wide range of patients,most of whom are critically ill.The condition of patients may advance at any time,and the mortality rate is relatively higher.Therefore,ICU nurses are required to have stronger professional ability and psychological quality.Such long-term heavy work will cause job burnout,and also have a certain impact on nurses’ willingness to stay on duty.Objective: This study observed the general situation of job burnout and retention intention of ICU nurses in Grade-A tertiary hospital by investigating their job burnout and retention intention,and analyzed the correlation and related influencing factors to improve the job burnout of nurses and formulating the retention strategy of nurses.Methods: In this study,we selected ICU nurses from five Grade-A tertiary hospitals in Liaoning Province from February 2018 to September 2018.Questionnaire Star was used to collect questionnaires online,and then the data were processed and analyzed.The questionnaire that each research object needs to complete includes the basic information questionnaire,the Job burnout scale and the nurses’ intention to stay questionnaire.After this study,a total of 535 questionnaires were collected,of which 22 were excluded,and 513 valid questionnaires were obtained,with an effective rate of95.8%.SPSS 20.0 software was used to analyze the data.For the comparison of job burnout and intention to stay with different individual characteristics,the mean±standard deviation(mean±SD)and constituent ratio were used for descriptive analysis,and the t-test,analysis of variance,and chi-square test were used for the difference analysis between groups.Rank correlation analysis was used to analyze the correlation between job burnout and intention to stay.Binary Logistic regression was used to explore the influencing factors of job burnout.The difference was considered statistically significant when the test level was α=0.05 and P<0.05.Results:1.The average score of emotional failure in the survey population was(22.98±12.60)points;The average depersonalization score was(9.22±7.07);The average score of personal accomplishment was 27.97±13.29 points.The results showed that 84.0 percent of respondents felt job burnout,compared with 42.1 percent for mild,17.3 percent for moderate and 24.6 percent for severe.After comparison,it was found that the degree of depersonalization was significantly higher than the conventional model,and the difference was statistically significant(P<0.05).2.Retention intention(OR=0.800,CI=0.733-0.868),marital status(P=0.018)and nursing knowledge(OR=1.591,CI=1.174-2.156)were the influencing factors of emotional failure;Retention intention(OR=0.755,CI= 6694-0.821),nature of employment(OR=0.513,CI=0.289-0.910)and age(OR=1.344,CI=1.027-1.760)were the influencing factors of depersonalization.Retention intention(OR=1.171,CI=1.076-1.276),marital status(P<0.001),nursing knowledge(OR=1.524,CI=1.130-2.056)and the number of night shift per month(OR=0.592,CI=0.431-0.813)were the factors influencing personal achievement.The above differences were statistically significant(P<0.05).3.The average score of nurses’ intention to stay was(21.36±2.426)points,and the total score was 14-26 points.The results showed that gender,position,nature of employment,number of night shift per month and reason of nursing choice had significant differences among the five factors,while there was no significant statistical significance for other characteristics.4.In the comparison of individual characteristics of different job burnout groups,emotional failure has statistical significance in terms of years engaged in nursing work,professional title,marital status,nursing understanding and employment reasons.Depersonalization is different among groups in terms of title,nature of employment and reason of employment.There were differences in personal achievement among groups in age,gender,years of work,title,marital status,number of night shifts,childbearing status and nursing knowledge.5.The retention intention of nurses with different burnout had statistical difference(P<0.001),and the score of retention intention of nurses with severe burnout was significantly lower than that of nurses with mild burnout and moderate burnout.6.The correlation between retention intention and job burnout was statistically significant(emotional exhaustion r=-0.275**,depersonalization r=-0.379**,personal sense of achievement r=0.280**,P<0.001).Emotional exhaustion was positively correlated with depersonalization(r=0.706**,P<0.001)and personal achievement(r=0.299**,P<0.001).Conclusion:Job burnout can affect job retention intention,and there is a certain correlation between job burnout and the retention intention.Job retention intention is a protective factor of job burnout.When the intention to stay is stronger,the degree of job burnout is lower.Therefore,corresponding measures can be taken to alleviate the job burnout of ICU nurses,enhance their willingness to stay on duty,enhance their enthusiasm for work,enhance the quality of nursing work,and solve the problem of nursing staff shortage.
