Objective: To observe the therapeutic effect of intravitreal injection of Conbercept diabetic macular edema(DME)in OCT patients,and to investigate the relationship between the therapeutic effect and systemic factors(glycosylated hemoglobin,total cholesterol,low density lipoprotein,creatinine,uric acid,mean platelet width,mean platelet volume)and morbidity Correlation of local ocular conditions(outer membrane integrity,ellipsoid band integrity).Methods: A total of 60 eyes of diabetic macular edema patients were included in this study.Optical coherence tomography(OCT)of diabetic macular edema can be divided into three groups: diffuse retinal thickening(sponge-1ike diffuse retinal thickening,DRT),macular edema sac sample(cystoid macular edema,,CME)and serous retinal detachment(serous retinal detachment,SRD),20 patients in each group.Blood samples of patients were collected and the local characteristics of affected eyes were observed.Treatment with 3+PRN(pro re nata)was given as a 0.05mg(0.01ml) intravitreal injection of Conbercept.Using a retrospective cohort study,Different contrast observation OCT classification DME by Conbercept heap glass body cavity injection before and after treatment for 2 weeks,1 month after operation,postoperative march the best corrected visual acuity(best correctred aeuity,BCVA),macular center concave retinal thickness(central Foveal thickness,CFT).To evaluate the efficacy of Conbercept in the treatment of different types of DME.Results: 1.The difference of preoperative BCVA among the three groups was significant(P < 0.05).The BCVA of the three groups before and after each time point was compared,and the difference was significant(P < 0.05).In pairwise comparison,BCVA of DRT group was significantly different from that of the other two groups at each time point after surgery(P < 0.05).There was no significant difference in BCVA between CME group and SRD group at each time point after surgery(P > 0.05).2.The preoperative CFT of the three groups was compared,and the difference was significant(P < 0.05).There were significant differences in CFT between the three groups at each time point before and after surgery(P < 0.05).Pairwise,there was significant difference in CFT 2 weeks after operation between DRT group and SRD group(P < 0.05),but no significant difference in CFT 2 weeks after operation between CME group and SRD group and DRT group(P > 0.05).The CFT of SRD group at 1 month and 3 months after surgery was significantly different from that of the other two groups(P < 0.05).There was no significant difference in CFT between the CME group and the DRT group at 1 month and 3 months after surgery(P > 0.05).3.There was no correlation between glycosylated hemoglobin,creatinine and uric acid and visual acuity 3 months after surgery(P>0.05).Total cholesterol,low density lipoprotein,PDW and MPV were correlated with visual acuity 3 months after surgery(P < 0.05).4.There was significant difference in total cholesterol among the three groups(P <0.05).Pairwise comparison among groups showed that SRD group had the highest total cholesterol,and the difference was significant(P < 0.05).There was no significant difference in total cholesterol between DRT group and CME group(P>0.05).There was significant difference in low density lipoprotein among the three groups(P <0.05).Pairwise comparison among groups showed that SRD group had the highest LDL,followed by CME group,and DRT group had the lowest,and the difference was significant(P < 0.05).5.There was significant difference in MPV among the three groups(P < 0.05).The MPV of SRD group was the highest and the difference was significant(P < 0.05).There was no significant difference in MPV between DRT group and CME group(P=0.554).There was significant difference in PDW among the three groups(P <0.05).Pairwise comparison between groups showed that PDW in SRD group was higher than that in the other two groups,and the difference was significant(P <0.05).There was no significant difference in PDW between DRT group and CME group(P < 0.05).6.The ellipsoid band and outer membrane were correlated with visual acuity 3 months after operation(P < 0.05).Compared with the three groups,the ellipsoid band and outer membrane in DRT group were different from the other two groups,and the difference was statistically significant(P < 0.05).Conclusions: The three OCT types of diabetic macular edema showed varying degrees of visual acuity gain and anatomical structure recovery after treatment with Conbercept.The optimal corrected visual acuity of the affected eye was improved after surgery,and the thickness of macular fovea retina was reduced.The treatment effect of DRT type DME was better than that of the other two groups.This may be related to the fact that the integrity of preoperative DRT-type DME ellipsoid band and outer membrane was better than that of the other two groups.DME is correlated with blood lipid,and SRD type DME may be correlated with total cholesterol and low density lipoprotein.DME is associated with platelet activation,and SRD-type DME may be related to average platelet distribution width and average platelet volume. |
