Correlation Between Leucine-rich α-2 Glycoprotein-1 And Diabetic Macroangiopathy In Patients With Type 2 Diabetes Mellitus

Objective:Serum leucine-rich alpha-2 glycoprotein-1(LRG1)levels in patients with type 2diabetes were measured by enzyme-linked immunosorbent assay to determine whether there is a correlation between LRG1 and macroangiopathy in type 2 diabetes.The study will present new clinical ideas for the dynamic observation,prevention and early treatment of diabetic macrovascular complications.Methods:1.From October 2021 to December 2022,ninety patients with confirmed diagnosis of T2 DM admitted to the Endocrinology department of Northern Jiangsu People’s Hospital were selected according to the inclusion criteria.Based on the presence of macroangiopathy,they were divided into type 2 diabetes macrovascular complication group(MVC group,60 cases)and simple type 2 diabetes group(SDM group,30 cases).In addition,thirty healthy individuals were enrolled as the health control group(HC group).2.The general data,previous disease history and smoking history of 120 subjects were collected,and blood samples were taken to measure and record FBG,Hb A1 c,FINS,TC,TG,HDL-C,LDL-C,serum LRG1,Body Mass Index(BMI),and HOMA-IR.SPSS 26.0 software was applied to compare the general data,biochemical data and serum LRG1 of the three groups.The study used Pearson and spearman to analyze the correlation between serum LRG1 and clinical indexes.Risk factors for macroangiopathy in T2 DM were analyzed by binary logistic regression.ROC analysis was applied to predict the optimal threshold for the decision of serum LRG1 on T2 DM macrovascular disease.Results:1.Three groups of gender,age,smoking history showed no difference in contrast(P>0.05),while diabetes duration,systolic and diastolic blood pressure,BMI had evidently statistical difference(P<0.05).The duration of diabetes disease,systolic blood pressure in the MVC group was higher than that in the SDM group and the HC group(P<0.05),but there was no difference in BMI and diastolic blood pressure between the MVC group and the SDM group(P>0.05).2.The FBG,Hb A1 c,HOMA-IR,and IMT were elevated in the MVC group compared with the SDM group(P<0.05);FBG,Hb A1 c,TG,Ty G,HOMA-IR,and IMT were increased in the MVC group compared with the HC group,while HDL-C levels were decreased(P>0.05).FBG,Hb A1 c,TG,Ty G,and HOMA-IR were ascended and HDL-C levels were declined in the SDM group compared with HC(P<0.05).3.The serum LRG1 levels in the MVC group,SDM group and HC group were26.51±11.00,16.58±8.58 and 8.40±5.54,which showed a rising trend from the HC group to the MVC group(P<0.001).MVC group had a significantly higher serum level than SDM and HC group(P<0.001),and SDM group exceeded HC group(P<0.001).4.Correlation analysis between serum LRG1 and clinical indexes showed that LRG1 was positively correlated with BMI,systolic blood pressure,diastolic blood pressure,FBG,Hb A1 c,TG,Ty G,HOMA-IR,IMT,negatively correlated with HDL-C.However,age,TC,LDL-C and FINS were not related to LRG1(P>0.05).5.Multiple linear regression analysis was performed with LRG1 as dependent variable and BMI,systolic blood pressure,diastolic blood pressure,FBG,Hb A1 c,TG,Ty G,HOMA-IR,IMT and HDL-C as independent variables.The conclusion indicated that HOMA-IR,IMT and BMI were the influencing factors of LRG1(β=0.285,0.243,0.183,P<0.05).6.Logistic Regression Analysis was conducted to show that Systolic Blood Pressure,Hb A1 c,and LRG1 as Risk Factors for Macrovascular Complications in Type 2Diabetes7.The ROC curve was used to predict the diagnostic value of serum LRG1 for T2 DM macroangiopathy.The results showed the area under the curve was 0.856,and the optimal cutoff value was 19.185,indicating that serum LRG1 has moderate accuracy for the diagnosis of T2 DM macroangiopathy.Conclusion:Serum LRG1 level is increased in simple T2 DM patients,especially in T2 DM macroangiopathy patients,suggesting that LRG1 may be closely related to the occurrence and development of T2 DM and T2 DM macroangiopathy.
