ObjectiveBased on the theory of protective motivation,the current situation of breast cancer screening in gynecological outpatients was investigated,and the factors influencing the behavior of breast cancer screening in gynecological outpatients were analyzed,so as to provide theoretical basis for future intervention by the theory of protective motivation to promote the implementation of opportunistic screening in gynecological outpatients.MethodConvenience sampling method was used to select 280 patients who visited the gynecological outpatient department of a third-class A hospital in Dalian City from November 2021 to March 2022 as the research objects.In this study,general data questionnaire,breast cancer Protection Motivation Scale and breast cancer awareness scale were used to investigate.SPSS25.0 software was used for statistical analysis,and descriptive statistics,t test,one-way analysis of variance,Chi-square test,correlation analysis and binary Logistic regression were used to analyze each factor,P<0.05 was considered statistically significant.Results1.The rate of Breast self-examination,Breast clinical examination,Breast ultrasound and Mammography was 51.8%,63.2%,66.8% and 27.5% in gynecological outpatients.2.The total score of the breast cancer awareness scale for gynecological outpatients was(72.54±12.17)points.Among the three dimensions,the score of breast cancer symptom was(10.26±3.39)points,the score of the obstacle factor to medical treatment was(14.38±4.46)points,and the score of the risk factor of breast cancer was(47.9±9.06)points.3.Among the six dimensions of Breast Cancer PMT in gynecological outpatients,the score of risk perception was(3.31±0.59).Severity perception score was(3.91±0.76),fear perception score was(3.16±0.74),reaction efficacy score was(3.49±0.54),reaction cost score was(2.57±0.77),and self-efficacy score was(3.46±0.60).4.Different age,marital status,place of residence,state of residence,education level,occupation,average monthly income,history of benign breast disease and awareness of breast cancer had an impact on the scores of various dimensions of motivation to protect breast cancer in gynecological outpatients.Different age,marital status,place of residence,state of residence,education level,occupation,average monthly income and history of benign breast diseases have an impact on whether gynecological outpatients participate in breast cancer screening.5.Binary logistic regression analysis showed that age,marital status,occupation,history of benign breast disease,risk perception,response cost and self-efficacy affected the breast cancer screening behavior of gynecological outpatients.The determination coefficient R2 = 0.384,and the prediction accuracy of the model was 85.7%.Conclusion1.Breast ultrasound is the most commonly used method for breast cancer screening in gynecological outpatients,while Mammography has a low utilization rate.2.The total average score of breast cancer awareness of gynecological outpatients was 72.54,which was at the medium level(47-77).The mastery of knowledge related to breast cancer of gynecological outpatients still needs to be further improved.The risk perception,severity perception,fear perception,reaction efficacy and self-efficacy of gynecological outpatients were all higher than the theoretical mean(3),and the reaction cost was lower than the theoretical mean(3).The overall protective motivation of gynecological outpatients for breast cancer was above the average level.3.Gynecological outpatients aged 40~49 years,married,employed,with a history of benign breast cancer,higher risk perception,higher self-efficacy score,and lower response cost score were more likely to accept screening behavior.PMT can well explain the formation mechanism of breast cancer screening behavior in gynecological outpatients.Intervention measures can be developed to promote the implementation of opportunistic breast cancer screening in gynecological outpatients under the guidance of PMT.
