Objective: This study compares ultrasound-guided right internal jugular vein puncture and catheterization with syringes on the short axis(referred to as right internal jugular vein puncture and catheterization)and ultrasound-guided right cephalo-brachial vein oblique axis puncture and catheterization without syringes(referred to as right cephalobrachial vein modified puncture and catheterization),and discusses the clinical effect and safety of right cephalo-brachial vein modified puncture and catheterization.Methods: A total of 232 patients who underwent right cephalo-brachial vein puncture and catheterization in our hospital from January 2022 to December 2022 were randomly divided into control group and improved group with 116 cases each.The control group adopted ultrasound-guided right internal jugular vein puncture and catheterization with syringes on the short axis(referred to as right internal jugular vein puncture and catheterization),and the improved group adopted ultrasound-guided right cephalobrachial vein oblique axis puncture and catheterization without syringes(referred to as right cephalo-brachial vein modified puncture and catheterization).The operation time,puncture success rate,puncture times,quality of life and puncture complications of the two groups were compared and analyzed.Results: The operation time and puncture times in the improved group were significantly lower than those in the control group(P;The first puncture success rate and total puncture success rate of the control group and the improved group were90.5%,94.0% and 94.8%,100%,respectively.The first puncture success rate and the first puncture success rate of the control group were lower than those of the improved group,with significant difference(P<0.05);One case of pneumothorax and one case of edema occurred in the improved group,and the total incidence of complications was1.7%.In the control group,4 cases of pneumothorax,3 cases of edema,and 2 cases of brachial plexus sensory abnormalities occurred,and the total incidence of complications was 7.8%.The incidence of complications in the improved group was significantly lower than that in the control group(P<0.05).Conclusion: Ultrasound-guided modified puncture and catheterization of right cephalic vein can effectively reduce the number of puncture,operation time and complications,reduce the discomfort of patients,and improve the success rate of puncture and the quality of life of patients.Its effectiveness and safety are higher than that of right internal jugular vein puncture and catheterization,and it has good clinical application value,especially for patients with difficulty in puncture,it has important clinical practical value,safety and reliability.
