Analysis Of Early Rehabilitation Effect Of Intramuscular Effect Bandage Combined With Resistance Training And Balance Training After Repair Of Anterior Talofibular Ligament Under Arthroscope

Objective:Total arthroscopy was used to enhance the anterior talofibular ligament for external ankle instability,and intramuscular effect patch combined with resistance and balance training was given in the postoperative rehabilitation stage.Compared with traditional rehabilitation programs,the efficacy and safety of early rehabilitation were discussed.Methods:A total of 47 patients who received enhancement repair of the anterior talofibular ligament under full microscope for the first time from January 2020 to December 2021 according to the inclusion and exclusion criteria in the Northern Theater Command General Hospital were randomly divided into the experimental group(24 cases)and the traditional group(23 cases).Both groups were fixed with postoperative support for 2 weeks,and the experimental group was changed to intramuscular effect paste combined with resistance training and balance sensation training 2 weeks later.The traditional group was given ankle pump exercises and continued wearing of the brace.Early postoperative general conditions,whether there were related complications,VAS score,AOFAS score,Kofoed score,CAIT score and single ankle heel lifting height of the affected limb at 6,8 and 10 weeks after rehabilitation training in both groups were recorded.Meanwhile,periankle muscle strength grading at 10 weeks after rehabilitation was recorded.Results:Satisfactory follow-up was obtained after operation,and a total of 1 case was lost to follow-up,and there were no statistically significant differences between the two groups in gender,age,course of disease,BMI,ankle muscle strength grade,injury side,etc.All patients had good surgical incision healing,and no complications such as ankle infection,intraoperative nerve injury,lower limb vein thrombosis,and anchor removal occurred.VAS scores in the early postoperative period decreased gradually in both groups,and there was no statistically significant difference between the two groups at the 3rd and 7th day after surgery.VAS scores in the rehabilitation stage showed statistically significant comparison between the two groups at each follow-up node,indicating better efficacy in the experimental group.The intragroup comparison showed that the scores of the two groups decreased gradually at the stage of rehabilitation.The experimental group recovered to the preoperative level at the 8th week of rehabilitation,while the VAS scores of the traditional group decreased slowly,and the scores were still higher at the 10 th week.AOFAS score and Kofoed score showed similar results,and pair comparisons in each time period of the two groups were statistically significant.At the 10 th week,scores in both groups reached the peak during the follow-up period.Comparison between groups showed that with the progress of rehabilitation training,at the 8th and 10 th week,experimental group showed better efficacy,and the difference was statistically significant(P<0.05).Inter-group comparison of CAIT evaluation showed that the experimental group was superior at 6 weeks,8 weeks and 10 weeks of rehabilitation(P<0.05).Intra-group comparison showed that CAIT score of the experimental group gradually improved and began to be effective at 8 weeks of rehabilitation,It peaked at week 10(24.04 ± 1.42 score),while the traditional group scored lower at 6 weeks of rehabilitation than before surgery(P<0.05),and only became effective at 10 weeks(22.13 ± 1.39 score).Compared with previous time points,the results were statistically significant(P<0.05).In the rehabilitation period,the comparison between the two groups was statistically significant,and the experimental group was better than the traditional group(P<0.05).The intra-group comparison showed that the average level of calf lifting height in the experimental group was higher than that before surgery at the 8th week(4.08±0.28cm),but the difference was not statistically significant,and the traditional group exceeded the preoperative level at the10 th week(4.51 ± 0.61cm).Pairwise comparison in any time period had statistical significance(P<0.05).At the 10 th week,the muscle strength grading of the affected ankle muscles in both groups was ≥ grade 3,the muscle strength grading of the experimental group was mainly grade 5,and the muscle strength grading of the traditional group was mainly grade 4,and there were significant differences in the muscle strength grading of the selected muscles(P<0.05),indicating that the muscle strength or stability of the affected ankle muscles in experimental group was better than traditional group.Conclusion:The application of intramuscular effect bandage combined with antiresistance balance training can help patients recover activities early,increase the muscle strength around the ankle joint,improve the stability of the ankle joint,improve the function of the ankle joint,relieve pain,without obvious safety risks,and is worthy of clinical promotion.
