| Objective : In order to understand the relationship between the incidence of stroke and spleen and stomach,and to study the correlation between the degree of neurological deficit of stroke and the function of spleen and stomach,the detailed stroke information collection and spleen and stomach function investigation were carried out in 204 patients with acute stroke.The etiology and pathogenesis of stroke were discussed from the theory of spleen and stomach in traditional Chinese medicine,so as to provide the basis for clinical selection of reasonable treatment plan and give full play to the curative effect of traditional Chinese medicine.Methods : Collect 204 patients who were diagnosed as acute stroke in the Department of Traditional Chinese Medicine Classics and Encephalopathy Center of Shandong Provincial Hospital of Traditional Chinese Medicine,Tai ’an Hospital of Traditional Chinese Medicine,Dezhou Hospital of Traditional Chinese Medicine,Rizhao Hospital of Traditional Chinese Medicine and Weifang Hospital of Traditional Chinese Medicine from December 2021 to September 2022.First of all,the basic information such as the time of onset,inducement,height and weight,BMI,clinical manifestations,tongue coating and pulse,related medical history,laboratory indicators and imaging examinations related to stroke,and treatment plan were asked in detail.At the same time,the history of spleen and stomach diseases,daily eating habits,smoking and drinking history were understood,and the NIHSS neurological function score and spleen and stomach function score were recorded respectively.Secondly,the information of all patients is input into the computer to establish a database.Finally,the number of cases of spleen and stomach diseases in patients with acute stroke was counted,and the difference between the overall spleen and stomach function and healthy adults was analyzed.The factors causing the damage of spleen and stomach function were studied,and the differences of spleen and stomach function between patients with different degrees of neurological deficit were compared.The correlation between the acute stage of stroke and the spleen and stomach function before the onset was studied.Results :1.Basic data : A total of 204 patients with acute stroke were collected,including 132males(64.70 %)and 72 females(35.30 %)aged 20-91 years,with an average age of63.99 ± 11.936 years.The average age of males was 61.31 ± 11.006 years old,and the average age of females was 68.90 ± 12.084 years old.There were 0 cases of viscera and204 cases of meridians,including 196 cases of ischemic stroke and 8 cases of hemorrhagic stroke.Of the ischemic patients,43 received thrombolytic therapy and 153 did not.2.Related medical history and inducement : The proportion of patients with hypertension,diabetes,coronary heart disease and hyperlipidemia before onset was 70.1 %,32.35 %,17.65 % and 6.37 %,respectively.The most common causes of stroke were fatigue and exogenous factors,accounting for 40.62 % and 26.65 % respectively.The proportion of diet and emotion was less,accounting for 17.19 % and 15.63 % respectively.3.Analysis of the causes of spleen and stomach disorders : the proportion of BMI being overweight or obese was 63.2 %;35.8 % of the patients were addicted to cold food,and 58.3 % were addicted to fat and greasy food.70 cases of smoking accounted for34.3 %,80 cases of drinking history accounted for 39.2 %;the proportion of male patients with smoking and drinking history was 51.6 % and 58.3 %,respectively.4.Among the 204 patients with acute stroke,75 had organic diseases of the spleen and stomach system,accounting for 36.8 %.The overall spleen and stomach function score of patients was significantly higher than that of healthy adults(p < 0.05).The degree of neurological deficits in patients increased with the increase of spleen and stomach function scores,and the difference was statistically significant(P < 0.05).Conclusion :1.About one-third of patients with acute stroke have spleen and stomach diseases,and compared with healthy adults,they have poor spleen and stomach function.The worse the spleen and stomach function,the more serious the neurological deficit;spleen and stomach disorders may be an important cause of stroke.2.Most of the patients with acute stroke have unhealthy living habits such as fat and sweet diet,smoking and drinking,which can lead to phlegm-dampness damage to the spleen and stomach and aggravate the burden of the spleen and stomach.3.BMI are associated with stroke. |
